信息化软件在医院环境卫生学监测闭环管理中的作用 被引量:13

The role of information software in closed-loop management of hospital environmental hygiene monitoring

摘要 目的探究信息化软件在医院环境卫生学监测中的作用,及环境卫生学监测结果与医院感染的关系,为科学感控提供指导。方法应用杏林环境卫生学专家系统信息化软件对杭州市第一人民医院环境卫生学监测进行闭环管理,收集软件使用前(2017年1月-2018年12月)和软件使用后(2019年1月-2020年6月)的空气、物体表面、手卫生、医疗用水、消毒后使用物品和消毒液等环境卫生学监测结果和同期医院感染发生率,探讨信息化软件对环境卫生学监测质量的影响,并进一步分析环境卫生学监测结果与医院感染发病率的相关性。结果 2017年1月-2020年6月,环境卫生学监测总体合格率为94.72%(5 112/5 397)。应用信息化软件后,监测合格率由使用前的93.93%(2 414/2 570)提高到使用后的95.44%(2 698/2 827)(P=0.013),任务逾期率由7.20%(185/2 570)降低到1.10%(31/2 827)(P<0.001),复查及时率由12.82%(20/156)提高到90.70%(117/129)(P<0.001)。随着环境卫生学监测合格率的上升,医院感染发病率呈逐渐下降的趋势(r=-0.502,P<0.001)。结论信息化软件的应用有助于提高医院环境卫生学监测的质量,通过多重闭环管理,为后续发现、识别、干预、控制医院感染高危因素提供依据,降低医院感染的发生。环境卫生学监测软件的即时闭环管理,可规范监测流程,提高监测效率,及时控制院感风险。 OBJECTIVE To investigate the role of information software in environmental hygiene monitoring, and the relationship between the results of environmental hygiene monitoring and hospital infection, so as to provide guidance for scientific infection prevention and control. METHODS The information software of Xinglin environmental hygiene expert system was used to conduct closed-loop management of hospital environmental hygiene monitoring in the Hangzhou First people’s Affiliated Hospital, and the environmental hygiene monitoring results of air, object surface, hand hygiene, medical water, disinfected articles and disinfectants, as well as the incidence of nosocomial infections during the same period were collected before the use of software(2017 January-2018 December) and after the use of(2019 January-2020 June). The impact of information software on the quality of environmental hygiene monitoring was discussed, and the correlation between the results of environmental hygiene monitoring and the incidence of hospital infection were further analyzed. RESULTS From January 2017 to June 2020, the overall qualified rate of environmental hygiene monitoring was 94.72%(5112/5397). After the application of information software, the qualified rate of monitoring was increased from 93.93%(2414/2570) before use to 95.44%(2698/2827) after use(P=0.013), the overdue rate of tasks was reduced from 7.20%(185/2570) to 1.10%(31/2827)(P<0.001), and the timely rate of reexamination was increased from 12.82%(20/156) to 90.70%(117/129)(P<0.001). With the increase in the qualified rate of environmental hygiene monitoring, the incidence of nosocomial infection was decreased gradually(r=-0.502, P<0.001). CONCLUSION The application of information software was helpful to improve the quality of hospital environmental hygiene monitoring, and to provide a basis for subsequent discovery, identification, intervention and control of high risk factors of nosocomial infection through multiple closed-loop management, so as to reduce the incidence of hospital infection. The real-time closed-loop management of environmental hygiene monitoring software can standardize the monitoring process, improve the monitoring efficiency, and control the hospital infection risk in time.
