Reasons and Improvement Plan of Drug Delivery Errors in Outpatient Dispensary of Infectious Diseases Hospital

目的探究某院医院门诊药房发药差错原因并提出改进方案。方法本次研究选取本院门诊药房从2019年6月—2020年6月发药差错事件中的80例,对发药差错事件进行回顾性分析,确定发生问题的事故类型、问题发生几率以及其发生原因,对问题提出针对性的改进方案。结果2019年6月—2020年6月本院门诊药房药品分发错误事件共80例,经调查与分析后结果显示,药品分发错误的类型主要为剂型分发错误、药品分发错误和药品剂量错误。门诊药房药品分发中药品分发错误、剂量分发错误、剂型分发错误分别占比47.5%、11.2%、36.2%,造成我院医院门诊药房药品发放错误的原因主要是电子处方的输入差错以及输入不规范、药品的包装、规格和剂型等问题,对于药品的摆放随意储存不当以及对于药品的管理制度较为混乱等,药品发放人员的专业素质也是重要影响因素。结论针对本院医院门诊药房发药差错原因,医院必须要将药品管理与发放制度进行完善,提高药品管理人员的综合素质以及工作责任心,提高药品服务质量。

Objective To explore the causes of drug delivery errors in outpatient dispensaries of infectious diseases hospitals and propose improvement plans.Methods In this study,80 cases of drug delivery errors from June 2019 to June 2020 in outpatient pharmacy of our hospital were selected to analyze the drug delivery errors retrospectively,determine the types of accidents,the probability of problems and the causes,and put forward targeted improvement plans for the problems.Results From June 2019 to June 2020,there were 80 cases of drug distribution errors in outpatient pharmacy of our hospital.After investigation and analysis,the results showed that the main types of drug distribution errors were dosage form distribution errors,drug distribution errors and drug dosage errors.The proportion of drug distribution errors,dosage distribution errors and dosage form distribution errors in outpatient pharmacy 47.5%,11.2%and 36.2%respectively.The main causes of drug distribution errors in outpatient dispensaries of infectious diseases hospitals are the input errors of electronic prescriptions,the irregular input,the packaging,specifications and dosage forms of drugs.The professional quality of drug distribution personnel is also an important factor in the improper storage of drugs and the disorder of drug management system.Conclusion In view of the causes of drug delivery errors in outpatient dispensaries of infectious diseases hospitals,the hospital must improve the drug management and distribution system,improve the comprehensive quality and sense of responsibility of drug managers,and improve the quality of drug services.
