盐酸氟西汀联合经颅直流电刺激治疗抑郁症的干预效果 被引量:4

摘要 目的探讨盐酸氟西汀配合经颅直流电刺激对抑郁症患者血脂水平及血清镁离子(Mg2+)Mg2+、同型半胱氨酸(Hcy)Hcy表达的干预效果。方法选取2016年3月~2019年3月我院114例抑郁症患者,简单随机化分为3组,各38例。对照A组予以盐酸氟西汀治疗,对照B组予以经颅直流电刺激治疗,观察组予以盐酸氟西汀配合经颅直流电刺激治疗。治疗6周后比较3组临床疗效、抑郁自评量表(SDS)、汉密顿抑郁量表(HAMD)、总胆固醇(TC)、甘油三酯(TG)、血清Mg2+、同型半胱氨酸(Hcy)、脑源性神经营养因子(BDNF)、神经特异性烯醇化酶(NSE)、胶质纤维酸性蛋白(GFAP)水平、蒙特利尔认知评估量表(MoCA)、生活质量评价量表(SF-36)评分。结果观察组治疗后总有效率(94.74%)高于对照A组(76.32%)、对照B组(73.68%),SDS、HAMD评分低于对照A、B组(P<0.05);观察组治疗后TC、TG、Mg2+、Hcy、NSE、GFAP水平低于对照A、B组(P<0.05);观察组治疗后,血清BDNF水平、MoCA、SF-36评分高于对照A、B组,NSE、GFAP水平低于对照A、B组(P<0.05)。;观察组治疗后MoCA、SF-36评分高于对照A、B组(P<0.05)3组治疗期间不良反应相比,无明显差异(P>0.05)。结论盐酸氟西汀配合经颅直流电刺激治疗抑郁症,可改善患者抑郁情绪与认知功能,提高患者生活质量,其机制可能与降低TC、TG、Mg2+、Hcy、NSE、GFAP水平及提高BDNF水平有关。 Objective To investigate the effect of fluoxetine hydrochloride combined with transcranial direct current stimulation on the blood lipid level and the expression of serum magnesium ion(Mg2+)and homocysteine(Hcy)in patients with depression.To investigate the effect of fluoxetine hydrochloride combined with transcranial direct current stimulation on blood lipid levels and serum Mg2+and Hcy expression in patients with depression.Methods From March 2016 to March 2019,114 patients with depression in our hospital were selected and simply randomized into three groups,each with 38 patients.The control group A was treated with fluoxetine hydrochloride,the control group B was treated with transcranial direct current stimulation,and the observation group was treated with fluoxetine hydrochloride combined with transcranial direct current stimulation.Six weeks after treatment,the clinical efficacy,self-rating depression scale(SDS),Hamilton depression scale(HAMD),total cholesterol(TC),triglyceride(TG),serum Mg2+,homocysteine(Hcy),brain-derived neurotrophic factor(BDNF),neurospecific enolase(NSE),glial fibrillary acidic protein(GFAP)levels,Montreal cognitive assessment scale(MoCA),quality of life assessment scale(SF-36)score were compared among the three groups.Results After treatment,the total effective rate in the observation group(94.74%)was higher than that in the control A group(76.32%)and the control B group(73.68%),the scores of SDS and HAMD were lower than the control groups A and B the scores of SDS and HAMD were lower than those of controls A and B(P<<0.05);.After treatment,the levels of TC,TG,Mg2+,Hcy,NSE,and GFAP in the observation group were lower than those in the control groups A and B,and the serum BDNF levels,MoCA and SF-36 scores were higher than those in the control groups A and B(P<0.05).There was no significant difference in adverse reactions in the three groups during the treatment period(P>0.05).after treatment,the levels of TC,TG,Mg2+,and Hcy in the observation group were lower than those in the control A and B groups(P<0.05);the serum BDNF levels in the observation group were higher than those in the control A and B groups,and the levels of NSE and GFAP were lower than those in the control A and B groups(P<0.05);the scores of MoCA and SF-36 in observation group were higher than those in control group A and B after treatment(P<0.05).Conclusion Fluoxetine hydrochloride combined with transcranial direct current stimulation in the treatment of depression can improve the depression and cognitive function of patients,and improve the quality of life of the patient.fluoxetine hydrochloride combined with transcranial direct current stimulation can improve the depressive mood and cognitive function,and improve the quality of life of patients.The mechanism may be related to the decrease of TC,TG,Mg2+,Hcy,NSE,GFAP and the increase of BDNF.
