摘要
目的:研究为进行椎管内麻醉手术的老年患者使用小剂量右美托咪定的临床效果及安全性。方法:选取2018年1月至2019年6月期间在江苏省常熟市第一人民医院进行椎管内麻醉手术的66例老年患者为研究对象。按照随机数表法将其等分为A组、B组和C组。术中,为A组22例患者使用氯化钠注射液,为B组22例患者使用小剂量的右美托咪定,为C组22例患者使用大剂量的右美托咪定。手术结束后,对比三组患者血压、心率、镇静评分、脑电双频指数的变化情况和用药不良反应的发生情况。结果:用药20 min、30 min、60 min后,C组患者的舒张压、收缩压、心率、镇静评分及脑电双频指数均低于A组及B组,P<0.05。用药期间,B组患者不良反应的总发生率低于A组及C组患者,P<0.05。结论:为进行椎管内麻醉手术的老年患者使用小剂量的右美托咪可有效地稳定其血压及心率,保障其麻醉的效果和安全性。
Objective:To investigate the clinical efficacy and safety of low-dose dexmedetomidine in elderly patients undergoing intravertebral anesthesia.Methods:From January 2018 to June 2019,66 elderly patients undergoing spinal anesthesia surgery in The First People’s Hospital of Changshu city were selected as study subjects.They were equally divided into Group A,Group B and group C according to the random number table method.Intraoperatively,22 patients in group A were treated with sodium chloride injection,22 patients in group B were treated with low-dose dexmedetomidine,and 22 patients in group C were treated with high-dose dexmedetomidine.After the operation,the changes of blood pressure,heart rate,sedation score,eeg dual-frequency index and the occurrence of adverse drug reactions were compared among the three groups.Results:After 20 min,30 min and 60 min,diastolic blood pressure,systolic blood pressure,heart rate,sedation score and dualfrequency index of EEG in group C were all lower than those in group A and B(P<0.05).During the medication period,the total incidence of adverse reactions in group B was lower than that in group A and C(P<0.05).Conclusion:Low dose dexmedetomidine can effectively stabilize blood pressure and heart rate and ensure the anesthetic effect and safety in elderly patients undergoing intravertebral anesthesia.
