自拟化瘀利水方治疗腰椎间盘突出症急性期临床观察

Clinical Observation of Self-made Huayu Lishui Formula on Patients with Lumbar Disc Herniation in Acute Stage

目的观察自拟化瘀利水方治疗腰椎间盘突出症急性期的临床疗效。方法将86例腰椎间盘突出症急性期患者随机分为治疗组与对照组各43例;两组均给予20%甘露醇静滴5 d,治疗组同时给予自拟化瘀利水方汤剂口服2周;对照组给予双氯芬酸钠肠溶片口服2周;分别于治疗前(T0)、治疗后5 d(T1)、10 d(T2)、14 d(T3)监测两组患者疼痛视觉模拟评分(VAS)、腰椎疾患综合成绩评分(JOA)和Oswestry功能障碍指数(ODI)评分情况。结果两组治疗后各时间点各评分均优于治疗前(P<0.05),治疗组的VAS评分及ODI评分持续减低,JOA评分持续增高,均较前一时间点差异有统计学意义(P<0.05);对照组各指标T3与T2时变化差异无统计学意义(P>0.05);T1时治疗组和对照组各指标评分之间差异无统计学意义(P>0.05),T2、T3时对照组各指标评分均优于对照组(P<0.05)。结论自拟化瘀利水方能明显减轻腰椎间盘突出症急性期疼痛症状,改善腰椎功能,疗效持续。

Objective:To observe the effects of self-made Huayu Lishui Formula in the treatment of patients with lumbar disc herniation(LDH)in acute stage.Methods:A total of 86 patients with LDH in acute stage were randomly divided into the treatment group and control group,43 cases in each group.Both groups were given 20%mannitol intravenously for 5 days,while the treatment group was given self-made Huayu Lishui Formula orally for 2 weeks;the control group was given diclofenac sodium enteric-coated tablets orally for 2 weeks.The visual ana⁃logue scale(VAS)score,the Japanese Orthopaedic Association(JOA)lumbar spine disorder comprehensive perfor⁃mance score and Oswestry dysfunction index(ODI)of the two groups at before treatment(T0),5 d(T1),10 d(T2),14 d(T3)after treatment were monitored.Results:The scores of the two groups at each time point after treatment were better than those before treatment(P<0.05).The VAS score and ODI score of the treatment group continued to decrease,and the JOA score continued to increase.There were statistical differences compared to the previous time point(P<0.05);there was no statistical difference in the changes of the indicators of the control group at T3 and T2(P>0.05);there was no statistical difference between the scores of the treatment group and the control group at T1(P>0.05),The scores of all indicators in the treatment group at T2 and T3 were better than those of the control group(P<0.05).Conclusion:Self-made Huayu Lishui Formula could relieve pain of lumbar disc her⁃niation in acute stage and improve lumbar function with continuous effect.
