两种输尿管支架对青中年女性行输尿管镜钬激光碎石术患者术后生活质量和性功能的影响

Effect of two ureteral stents on the quality of life and sexual function after ureteral holmium laser lithotripsy in young and middle-aged female patients

目的探讨青中年女性行输尿管镜钬激光碎石术患者术后留置不同的输尿管支架对生活质量和性功能的影响。方法选取2015年10月至2019年3月江苏大学附属金坛医院泌尿外科诊治的94例输尿管中下段结石并拟行输尿管镜钬激光碎石术的青中年女性患者作为研究对象。根据随机留置的支架管分为双J管组和Polaris loop组,比较两组患者围手术期基线资料,记录不良事件及术后随访情况。结果两组患者术前基线资料比较,差异无统计学意义(P>0.05)。留置支架期间,Polaris loop组患者炎症指标、下尿路症状(LUTS)、排尿时腰疼情况、生活质量评分(QOL)、视觉模拟评分法(VAS)评分、女性性功能指数(FSFI)评分均优于双J管组,差异具有统计学意义(P<0.05)。结论输尿管镜钬激光碎石术后留置输尿管支架影响青中年女性患者的生活质量和性功能。Polaris loop支架具有独特的鱼尾状设计,与传统双J管相比,对女性患者术后的生活质量和性功能干扰较小。

Objective To investigate the effect of two kinds of ureteral stenting on the quality of life and sexual function after ureteralholmium laser lithotripsy in young and middle-aged women.Methods A total of 94 female patients with middle and lower ureteral calculi diagnosed and treated in Department of Urology of Jintan People′s Hospital affiliated to Jiangsu University from October 2015 to March 2019,who were scheduled to undergo ureteroscopic holmium laser lithotripsy,were selected as research objects.The patients were divided into the double-J stenting group and the Polaris loop stenting group,according to the kinds of stent.The perioperative baseline data of the two groups were compared,and indicators such as adverse events and postoperative follow-up results were recorded.Results Comparison of preoperative baseline data between the two groups showed no statistically significant difference(P>0.05).During the period of indwelling stents,the Polaris loop stenting group outperformed the double-J stenting group in inflammation index,lower urinary tract symptoms(LUTS),backache during urination,quality of life score(QOL),visual analogue pain scale(VAS)and other relevant clinical indicators.During the indwelling stenting period,the female sexual function index(FSFI)score of Polaris loop stenting group was better than that of double J stenting group,and the difference was statistically significant(P<0.05).Conclusions Ureteral stenting affects the quality of life and sexual function of young and middle-aged famale patients after ureteroscopic holmium laser lithotripsy.Compared with the traditional double-J stent,Polaris loop stent has a unique fishtail design and less interference on the quality of life and sexual function of patients after operation.
