咪喹莫特联合艾拉光动力疗法治疗尖锐湿疣的效果及对其相关血清指标水平的影响 被引量:5

Effect of Imiquimod combined with Ella photodynamic therapy on condyloma acuminatum and its influence on serum index level

摘要 目的探讨咪喹莫特联合艾拉光动力疗法治疗尖锐湿疣的效果及对T淋巴细胞亚群的影响。方法选取2018年1月至2019年1月于湖南省人民医院接受治疗的100例尖锐湿疣患者作为研究对象。随机分为光动力疗法组(n=50)和联合治疗组(n=50),光动力疗法组患者采用艾拉光动力疗法进行治疗,联合治疗组患者采用咪喹莫特联合艾拉光动力疗法进行治疗。统计并比较两组患者创面结痂、愈合时间;使用免疫透射比浊法检测血管内皮生长因子(VEGF)、缺氧诱导因子-1α(HIF-1α)水平;使用酶联免疫吸附试验法检测血清白细胞介素-10(IL-10)、白细胞介素-4(IL-4)、肿瘤坏死因子-α(TNF-α)水平;使用Western blot法检测Bcl-2、Bax蛋白相对表达量;使用流式细胞仪检测T淋巴细胞亚群水平;比较两组患者的治疗效果。结果联合治疗组患者创面结痂、愈合时间均短于光动力疗法组,差异具有统计学意义(P<0.05)。治疗后,联合治疗组患者血清VEGF、HIF-1α、IL-10、IL-4、TNF-α水平均低于光动力疗法组,差异具有统计学意义(P<0.05)。治疗后,联合治疗组患者Bcl-2表达低于光动力疗法组,Bax表达高于光动力疗法组,差异具有统计学意义(P<0.05)。治疗后,联合治疗组患者CD3^+、CD4^+水平高于光动力疗法组,CD8^+水平低于光动力疗法组,差异具有统计学意义(P<0.05)。联合治疗组患者治疗总有效率高于光动力疗法组,差异具有统计学意义(P<0.05)。结论使用咪喹莫特联合艾拉光动力疗法对尖锐湿疣患者进行治疗,能够加快患者创面愈合,抑制患者炎症反应以及疣体发展,改善患者免疫功能,治疗效果显著。 Objective To investigate the effect of Imiquimod combined with Ella photodynamic therapy on condyloma acuminatum and its effect on T lymphocyte subsets.Methods 100 patients with condyloma acuminatum treated in Hunan People′s Hospital from January 2018 to January 2019 were selected,and randomly divided into photodynamic therapy group(n=50)and the combined treatment group(n=50).Photodynamic therapy group were treated by Ella photodynamic therapy,while combined treatment group were treated by Imiquimod combined with Ella photodynamic therapy.The scab and healing time were calculated and compared between the two groups.The levels of vascular endothelial growth factor(VEGF)and hypoxia-inducible factor-1α(HIF-1α)were measured by immunotransmittance turbidimetry.Serum levels of interleukin-10(IL-10),interleukin-4(IL-4)and tumor necrosis factor-(TNF-)were detected by elisa.Western blot was used to detect the relative expressions of Bcl-2 and Bax proteins.T lymphocyte subsets were detected by flow cytometry.The therapeutic effects of the two groups were compared.Results Scab formation and healing time were shorter in the combined treatment group than in the photodynamic therapy group(P<0.05).After treatment,serum levels of VEGF、HIF-1α、IL-10、IL-4、TNF-αin the combined treatment group were lower than those in the photodynamic therapy group,and the differences were statistically significant(P<0.05).Bcl-2 expression in the combined treatment group was lower than that in the photodynamic therapy group,and Bax expression was higher than that in the photodynamic therapy group,with statistically significant differences(P<0.05).The levels of CD3^+and CD4^+in the combined treatment group were higher than those in the photodynamic therapy group,and the level of CD8^+was lower than that in the photodynamic therapy group,with statistically significant differences(P<0.05).The total effective rate of the combined treatment group was higher than that of the photodynamic therapy group,with statistically significant difference(P<0.05).Conclusions Imiquimod combined with Ella photodynamic therapy for the treatment of patients with condyloma acuminatum,can accelerate the wound healing,inhibit the inflammatory reaction of patients and the development of warts,improve the immune function of patients,the treatment effect is significant.
