中国肠造口患者性功能和/或性生活质量的系统评价

Sexual function and/or the quality of sexual life in Chinese enterostomy patients:a systematic review

目的通过系统评价的方法,分析中国肠造口患者性功能障碍的发生率,探讨性功能和/或性生活质量的影响因素。方法计算机检索PubMed、Cochrane Library、EMbase、CNKI、VIP、CBM和WanFang Data,搜集有关中国肠造口患者性功能和/或性生活质量的研究,由2名研究者独立筛选文献、提取资料及文献质量评价后,对纳入研究进行定性评价。结果共纳入38篇研究,以RCT为主(22篇),包括2702例患者。研究共涉及17种量表,目前尚未发现权威、统一的用于评估肠造口患者性功能和/或性生活质量的特异性量表。结果显示性功能障碍的发生率为7.14%(1/14)~100.00%(82/82),性生活满意度为0.00%(0/82)~76.67%(23/30)。家庭所在地、婚姻状况、家庭关系、社会支持、造口术后时间、手术方式、造口类型、有无并发症、自理能力、性生活指导、性生活指导需求和心理状态可能是肠造口患者性功能和/或性生活质量的影响因素。结论中国肠造口患者的性功能障碍发生率高且不同研究差异较大。应重视肠造口患者的性生活,建议未来开展更多大样本、多中心、高质量的研究。同时,构建适合用于中国肠造口患者性功能和性生活质量的特异性量表也是将来的工作方向之一。

Objective To analyze the incidence of sexual dysfunction and and to explore the influencing factors of sexual function and/or quality of sexual life in Chinese enterostomy patients by systematic review.Methods This paper searched PubMed,Cochrane Library,EMbase,CNKI,VIP,CBM and WanFang Data to collect studies on sexual function and/or quality of sexual life in Chinese enterostomy patients.Two researchers independently selected the literature,extracted the Data and evaluated the quality of the literature,and then made qualitative evaluation on the included studies.Results A total of 38 studies were included,with RCT as the dominant(22 papers),including 2,702 patients.The study involved 17 scales,and no definitive and uniform specific scales have been found to assess sexual function and/or quality of sexual life in enterostomy patients.The results showed that the incidence of sexual dysfunction ranged from 7.14%(1/14)to 100.00%(82/82),and sexual satisfaction ranged from 0.00%(0/82)to 76.67%(23/30).Homeplace,marital status,family relationship,social support,postoperative time,operation modes,colostomy types,stoma complications,self-care ability,sexual life instruction,demand for sexual life instruction and psychological conditiocn may be the influencing factors for sexual function and/or quality of sexual life in patients with enterostomy.Conclusions The incidence of sexual dysfunction in Chinese patients with enterostomy is high and different studies vary greatly.Attention should be paid to the sex life of patients with enterostomy,and it is suggested that more large-sample,multi-center and high-quality studies should be carried out in the future.Meanwhile,the construction of a specific scale suitable for the sexual function and sexual quality of Chinese enterostomy patients is also one of the future directions of work.
