多囊卵巢综合征肥胖患者血清铁蛋白与糖脂代谢的相关性分析

Correlation analysis of serum ferritin with carbohydrate and lipid metabolism in obese patients with polycystic ovary syndrome

目的探讨多囊卵巢综合征(PCOS)肥胖患者血清铁蛋白水平与胰岛素抵抗及糖脂代谢指标之间的相关性。方法选取2018年10月至2019年9月该院肥胖和糖尿病专科住院的84例肥胖女性为研究对象,根据是否患有PCOS分为PCOS组和对照组,每组42例。比较两组血清铁蛋白水平差异及分析其与糖脂代谢相关指标和性激素指标之间的相关性。结果PCOS组血清铁蛋白、睾酮、空腹胰岛素、C-肽、胰岛素抵抗指数、载脂蛋白E水平较对照组显著升高,差异有统计学意义(P<0.05);两组三酰甘油、总胆固醇、载脂蛋白A1、载脂蛋白B、糖化血红蛋白等指标差异无统计学意义(P>0.05)。Spearman相关分析显示血清铁蛋白水平与空腹胰岛素、C-肽、胰岛素抵抗指数、载脂蛋白E水平呈正相关(P<0.05),与年龄、体重指数、腰臀比、睾酮等无统计学关联(P>0.05)。结论PCOS肥胖女性血清铁蛋白水平升高,且与糖脂代谢及胰岛素抵抗相关,可能参与PCOS的发生、发展。

Objective To investigate the correlation of serum ferritin with insulin resistance and metabolic indicators of carbohydrate and lipid in obese patients with polycystic ovary syndrome(PCOS).Methods A total of 84 obese women hospitalized in the obesity and diabetes specialty department of the hospital from October 2018 to September 2019 were selected and divided into the PCOS group and control group according to whether having PCOS,42 cases in each group.The difference of serum ferritin level was compared between the two groups and the correlation between serum ferritin with glucolipid metabolism and sex hormone indexes was analyzed.Results The levels of ferritin,testosterone,fasting insulin,C peptide,homeostasis model assessment insulin resistance index(HOMA-IR)and apolipoprotein E in the PCOS group were significantly increased compared with those in the control group,and the differences were statistically significant(P<0.05).There was no statistically significant differences in the indicators of triglyceride,total cholesterol,apolipoprotein A1,apolipoprotein B and glycosylated hemoglobin between the two groups(P>0.05).The Spearman correlation analysis showed that serum ferritin level was positively correlated with th elevels of fasting insulin,C peptide,HOMA-IIR and apolipoprotein E(P<0.05),but had no correlation with the age,BMI,waist to hip ratio and testosterone(P>0.05).Conclusion Serum ferritin level is significantly increased in obese women with PCOS and is correlated to insulin resistance glucolipid metabolism,which may be involved in the pathogenesis and development of PCOS.
