椎体压缩性骨折椎体成形术后新发骨折与胰岛素抵抗的相关性

Correlation between new fractures after vertebroplasty and insulin resistance in vertebral compression fractures

目的探讨椎体压缩性骨折椎体成形术后新发骨折与胰岛素抵抗(IR)的相关性。方法选取2016年9月至2018年9月本科收治的98例椎体压缩性骨折椎体成形术患者作为研究对象,根据患者术后是否再发骨折将其分为再发骨折组34例和无骨折组64例,评估胰岛素抵抗指数(HOMA-IR),采用X线吸收仪测定骨密度T值,同时根据测量体质量和身高计算BMI值。结果再发骨折组患者HOMA-IR显著高于无骨折组(P<0.05),骨密度T值低于无骨折组(P<0.05);经Pearson相关性分析可见,再发骨折发生率与HOMA-IR呈正相关性(P<0.05);与骨密度T值呈负相关性(P<0.05)。结论椎体压缩性骨折椎体成形术后,患者骨折断端预后效果可受胰岛素抵抗指数影响,临床可将该指标作为预判风险因素,参考该指数变化情况开展相关预防性和干预性临床措施。

Objective To explore the correlation between new fractures after vertebroplasty and insulin resistance(IR)in vertebral compression fractures.Methods To select 98 cases of patients who underwent vertebral compression fracture vertebroplasty and were admitted to this department from September 2016 to September 2018 as the study objects,and they were divided into recurrent fracture group with 34 cases and no fractures group of 64 cases according to whether they had recurred fractures after operation to assess the insulin resistance index(HOMA-IR),and determine the T value of bone mineral density by using X-ray absorptiometry,and calculate the BMI value based on the measured body mass and height.Results The HOMA-IR in the recurrent fracture group was significantly higher than that in the no fractures group(P<0.05),and the T value of bone mineral density was lower than that in the no fractures group(P<0.05).Pearson correlation analysis showed that the incidence of recurrent fractures was positively correlated with HOMA-IR(P<0.05),and it was negatively correlated with the T value of bone mineral density(P<0.05).Conclusion After vertebroplasty for vertebral compression fractures,the prognosis of patients with fracture ends can be affected by the insulin resistance index.Clinically,this index can be used as a predictive risk factor,and relevant preventive measures and interventional clinical trials can be carried out with reference to the changes of the index.
