腰椎术后脑脊液漏患者两种不同引流护理的效果分析

Effect analysis of two different drainage nursing in patients who underwent cerebrospinal fluid leakage after lumbar surgery

目的探讨腰椎术后脑脊液漏患者不同的引流护理措施的效果。方法选择2017年1月至2018年12月在本院脊柱外科进行腰椎手术后出现脑脊液漏的患者46例,按组间基本特征具有可比性的原则分为A组26例和B组20例,A组采用负压引流护理,B组采用常压引流护理,比较两组患者在引流总量、引流时间、卧床时间、患者满意度、低颅压发生率、血肿发生率、切口延迟愈合、压力性损伤、深静脉血栓、泌尿系感染以及住院时间。结果两组患者在引流量、引流时间、低颅压症状的发生、患者满意度、卧床时间及住院时间比较差异有统计学意义(P<0.05),而在切口延迟愈合、压力性损伤、深静脉血栓、泌尿系感染及血肿发生率比较差异无统计学意义(P>0.05)。结论针对腰椎术后脑脊液漏的患者采用常压引流护理可预防低颅压症状发生、减少引流量、缩短卧床时间等,提高患者满意度,优于负压引流护理。

Objective To explore the effects of different drainage nursing measures in patients with cerebrospinal fluid leakage after lumbar surgery.Methods We select 46 cases of patients with cerebrospinal fluid leakage after lumbar surgery in our hospital from January 2017 to December 2018,and they were divided into group A with 26 cases and group B with 20 cases based on the principle of comparability of basic characteristics between groups,group A used negative pressure drainage care,and group B used normal pressure drainage care to compare the total drainage volume,drainage time,time in bed,patients'satisfaction,incidence of low intracranial pressure,incidence of hematoma,delayed incision healing,stress injury,deep vein thrombosis,urinary tract infection,and length of hospital stay.Results The two groups of patients had statistically significant differences in drainage volume,drainage time,the occurrence of low intracranial pressure symptoms,patients'satisfaction,time in bed and the length of hospitalization(P<0.05),but there was no statistically significant difference in the,delayed incision healing,stress injury,deep vein thrombosis,urinary tract infection,urinary tract infection and incidence of hematoma(P>0.05).Conclusion For patients with cerebrospinal fluid leakage after lumbar surgery,normal pressure drainage nursing can prevent the occurrence of low intracranial pressure symptoms,reduce drainage volume,shorten time in bed,etc.,and improve patients'satisfaction,which was better than negative pressure drainage nursing.
