摘要
背景:近年我国溃疡性结肠炎(UC)的患病率明显增高,维生素D3可能与其发生、发展密切相关。目的:探讨维生素D3联合美沙拉嗪对UC患者肠黏膜氧化应激损伤的修复作用。方法:选取2019年1月—2020年1月新疆医科大学第一附属医院确诊的120例轻-中度UC患者,并随机分为治疗组(n=60)和对照组(n=60),治疗组采用维生素D3联合美沙拉嗪治疗,对照组采用美沙拉嗪单药治疗。观察两组治疗8周后临床表现,比较治疗前后血清氧化应激[氧化型低密度脂蛋白(ox-LDL)和过氧化脂质(LPO)]、肠黏膜屏障受损情况[血清降钙素原(PCT)和二胺氧化酶(DAO)]和内镜下Mayo评分。结果:治疗8周后,治疗组的总有效率显著高于对照组(93.3%对78.3%;χ^2=5.551,P<0.05);两组血清ox-LDL、LPO、PCT、DAO水平以及Mayo评分均较治疗前显著下降(P<0.05),且治疗后治疗组上述指标水平显著低于对照组(P<0.05)。结论:维生素D3联合美沙拉嗪治疗对轻-中度活动期UC患者的疗效显著,能降低血清ox-LDL、LPO、PCT和DAO水平,对肠黏膜氧化应激损伤具有良好的修复作用。
Background:The prevalence of ulcerative colitis(UC)in China increased significantly in recent years.Vitamin D3 might be closely correlated with the development and progress of UC.Aims:To investigate the role of vitamin D3 combined with mesalazine in reparation of oxidative stress injury of intestinal mucosa in patients with UC.Methods:A total of 120 patients with mild or moderate UC from January 2019 to January 2020 at the First Affiliated Hospital of Xinjiang Medical University were enrolled,and were randomly divided into treatment group(n=60)and control group(n=60).Patients in the treatment group were treated with vitamin D3 combined with mesalazine,and patients in the control group were treated with mesalazine alone.Eight weeks after treatment,the clinical manifestations were observed.Serum oxidative stress[oxidized low density lipoprotein(ox-LDL)and lipid peroxidase(LPO)],intestinal mucosal barrier injury[serum procalcitonin(PCT)and diamine oxidase(DAO)]and Mayo score in the two groups were compared.Results:After 8 weeks treatment,the total effective rate was significantly higher in treatment group than in control group(93.3%vs.78.3%;χ^2=5.551,P<0.05);serum ox-LDL,LPO,PCT and DAO levels and Mayo score in the two groups were significantly decreased(P<0.05),and the above-mentioned indices in treatment group were significantly decreased than those in control group after the treatment(P<0.05).Conclusions:Vitamin D3 combined with mesalazine has reliable curative effect on mild and moderate UC patients,which can obviously reduce serum ox-LDL,LPO,PCT and DAO levels,and can repair the oxidative stress injury of intestinal mucosa.
