摘要
目的应用彩超二维斑点追踪成像超声技术计算起搏后左室各节段收缩时间差异,比较右室不同部位起搏后对左室收缩同步性的影响。方法选择2017年1月至2018年12月在本院植入双腔起搏器的患者70例,根据心室电极植入部位分为右心室心尖部(RVA)组和右心室流入道(RVIS)组,各35例。两组术前及术后均应用二维斑点追踪成像技术,测量左室横向应变达峰时间的最大差。结果起搏术前,两组左心室横向应变达峰时间的最大差(TTS-rg)及其最大差标准差(TTS-std)对比,无显著差异(P> 0.05)。起搏术后,RVIS组TTS-rg、TTS-std显著低于RVA组,差异显著(P <0.001)。结论 RVIS起搏较RVA起搏有更好的左室收缩同步性。
Objective To calculate the differences of left ventricular systolic time after pacing using color doppler twodimensional speckle tracking imaging(CDFI),to compare the effect of pacing in different parts of the right ventricle on left ventricular systolic synchrony.Methods From January 2017 to December 2018,70 patients with dual-chamber pacemaker were divided into right ventricular apex(RVA)group and right ventricular inflow tract(RVIS)group according to the implantation site.Before and after operation,two-dimensional speckle tracking imaging technique was used to measure the maximum difference of the peak time of lateral strain of left ventricle.Results Before pacing,there was no significant difference between the two groups(P>0.05)in the peak time of transverse strain of left ventricle(TTS-rg)and the standard deviation of the maximum deviation(TTS-std).After pacing,TTS-rg and TTS-std in RVIS group were significantly lower than those in RVA group(P<0.001).Conclusion RVIS pacing has better left ventricular systolic synchronism than RVA pacing.
