脑电图联合DWI及实验室检查的动态变化对CO中毒迟发脑病的预测研究

摘要 目的探讨MR扩散加权成像(DWI)联合脑电图及实验室检查的动态变化对一氧化碳(CO)中毒迟发脑病(DEACMP)的预测作用。方法回顾分析本院2016年4月至2018年12月收治的CO中毒迟发脑病患者80例,按检测方式分为对照组(40例接受脑电图检查)和研究组(40例接受脑电图联合DWI、血清NSE、血清S100β蛋白指标检测),比较分析两组患者疾病检出率、死亡率、并发症发生率、住院时间。结果研究组患者检出率(92.50%)显著高于对照组(70.00%),并发症发生率(7.50%)显著低于对照组(30.00%),死亡率(5%)显著低于对照组(25.00%),研究组住院时间(12.30±1.21) d显著短于对照组(25.65±1.24) d,差异均显著(P <0.05)。结论临床预测CO中毒迟发脑病可采用脑电图联合DWI,并测定其血清NSE、血清S100β蛋白指标的方式进行判定,此方式比单一使用脑电图方式检查的准确性更高,且对减少并发症、改善疾病预后有重要作用。 Objective To explore the predictive value of MR diffusion weighted imaging(DWI),electroencephalogram(EEG)and laboratory examination on delayed encephalopathy(DEACMP)after carbon monoxide(CO)poisoning.Methods From April 2016 to December 2018,80 patients with delayed encephalopathy due to CO poisoning were analyzed retrospectively,the patients were divided into control group(40 patients received EEG)and study group(40 patients received EEG combined with DWI,serum NSE and serum S100βprotein)by the way it's tested,the disease detection rate,mortality rate,complication rate and hospitalization time were compared between the two groups.Results The detection rate(92.50%)in the study group was significantly higher than that in the control group(70.00%),the complication rate(7.50%)was significantly lower than that in the control group(30.00%),and the mortality rate(5%)was significantly lower than that in the control group(25.00%),the duration of hospitalization in the study group(12.30±1.21)days was significantly shorter than that in the control group(25.65±1.24)days,the difference was significant(P<0.05).Conclusion EEG combined with DWI and serum NSE and S100βprotein can be used to predict the delayed encephalopathy after CO poisoning.This method is more accurate than EEG alone,it also plays an important role in reducing complications and improving the prognosis of diseases.
