摘要
目的探讨"1+1+1"家庭医生签约服务模式对社区老年高血压患者服药依从性及血压控制达标的影响,进一步规范社区老年高血压的健康管理。方法选取在社区进行治疗管理的1 040例老年高血压患者,随机分为签约组620例、对照组420例。对照组实施常规健康管理和社区治疗;签约组在对照组干预基础上,签订"1+1+1"家庭医生服务协议,对其进行有针对性的个体化干预。干预1个月、2个月、3个月后,观察两组患者服药依从性及血压控制达标情况。结果两组干预1个月、2个月、3个月比较,收缩压(SBP)签约组低于对照组,差异有统计学意义(P<0.05);舒张压(DBP)差异不显著(P> 0.05);签约组服药依从性、生活方式较对照组明显改善(P <0.05);签约组血压3个月内控制达标率优于对照组(P <0.05)。结论通过"1+1+1"家庭医生签约服务,有效提供个体化干预,社区老年高血压患者服药依从性提高,生活方式改善,血压控制达标率提升。
Objective To explore the influence of"1+1+1"family doctor contract service mode on the compliance of medication and blood pressure control of elderly hypertension patients in the community,and further standardize the health management of elderly hypertension in the community.Methods 1,040 elderly patients with hypertension were randomly divided into two groups,620 in the contract group and 420 in the control group.In the control group,routine health management and community treatment were carried out;on the basis of the intervention in the control group,the signing Group signed a"1+1+1"family doctor service agreement to carry out targeted individual intervention.After one month,two months and three months of intervention,the compliance of the two groups and the blood pressure control were observed.Results The systolic blood pressure(SBP)was significantly lower than that of the control group(P<0.05).There was no significant difference in DBP(P>0.05);the compliance and lifestyle of the signing group were significantly improved compared with the control group(P<0.05);the blood pressure control rate of the signing group within 3 months was better than that of the control group(P<0.05).Conclusion Through the"1+1+1"family doctor contract services,it can offer effective provision of individual intervention,with high hypertension drug compliance,lifestyle improvement,improving blood pressure control rate.
