子宫内膜异位症腹腔镜手术后地屈孕酮联合戈舍瑞林治疗对血清性激素及妊娠率的影响

Effects of duphaston combined with goserelin treatment on serum sex hormones and pregnancy rate after laparoscopic surgery for endometriosis

目的探究子宫内膜异位症腹腔镜手术后地屈孕酮联合戈舍瑞林治疗对血清性激素及妊娠率的影响。方法选取64例子宫内膜异位症患者(2018年7月~2019年12月),按照随机数字表法分为研究组(n=32)和对照组(n=32)。两组均行腹腔镜手术治疗,对照组术后采用戈舍瑞林治疗,研究组术后采用地屈孕酮联合戈舍瑞林治疗。比较两组妊娠率、不良反应发生率、治疗前后性激素水平[卵泡刺激素(FSH)、雌二醇(E2)、促黄体生成素(LH)]。结果治疗后两组血清FSH、E_2、LH水平均较治疗前降低,且研究组FSH低于对照组,E_2、LH高于对照组(P <0.05);研究组不良反应发生率9.38%低于对照组40.63%(P <0.05);两组妊娠率比较无明显差异(P> 0.05)。结论子宫内膜异位症患者腹腔镜手术后采用地屈孕酮联合戈舍瑞林治疗可有效调节性激素水平,降低不良反应发生率,且不影响妊娠率。

Objective To explore the effect of duphaston combined with goserelin treatment on serum sex hormones and pregnancy rate after laparoscopic surgery for endometriosis.Methods A total of 64 patients with endometriosis in our hospital(from July 2018 to December 2019)were selected and divided into study group(n=32)and control group(n=32)according to the random number table method.Both groups were treated with laparoscopic surgery,the control group was treated with goserelin after surgery,and the study group was treated with duphaston combined with goserelin after surgery.The pregnancy rate and the incidence of adverse reactions,levels of sex hormones before and after treatment[follicle stimulating hormone(FSH),estradiol(E2),luteinizing hormone(LH)]were compared between the two groups.Results After treatment,serum FSH,E2 and LH levels in the two groups were lower than before treatment,and FSH in the study group was lower than that in the control group,and E2 and LH were higher than in the control group(P<0.05);the incidence of adverse reactions in the study group was 9.38%lower than that in the control group,40.63%(P<0.05);there was no significant difference in pregnancy rates between the two groups(P>0.05).Conclusion The treatment of duphaston combined with goserelin after laparoscopic surgery in patients with endometriosis can effectively regulate the level of sex hormones,reduce the incidence of adverse reactions,and does not affect the pregnancy rate.
