早期综合护理对恶性肿瘤动脉化疗栓塞术后并发下肢深静脉血栓的影响及临床护理效果研究

Effects of Early Comprehensive Nursing on Lower Extremity Deep Venous Thrombosis after Arterial Chemoembolization for Malignant Tumors

目的探讨早期综合护理对恶性肿瘤患者动脉化疗栓塞术后并发下肢深静脉血栓(LEDVT)的影响及临床护理效果。方法选择我科2017年1月—2019年3月收治的160例恶性肿瘤动脉化疗栓塞术后患者为研究对象,采用随机数字表法分为对照组(73例)和观察组(87例)。对照组给予常规护理,观察组给予早期综合护理。比较两组患者护理后LEDVT、肺栓塞等并发症的发生率,焦虑及抑郁情绪、生活质量改善以及护理满意度的情况。结果观察组患者护理后LEDVT、肺栓塞、穿刺点感染、穿刺点出血等并发症的发生率均明显低于对照组(P<0.05);护理前,两组患者焦虑、抑郁、躯体、情绪、社会及疼痛功能评分比较,差异均无统计学意义(P>0.05);护理后,两组患者焦虑、抑郁、疼痛功能评分均明显降低,且观察组显著低于对照组(P<0.05),两组患者躯体、情绪、社会功能评分均显著升高,且观察组显著高于对照组(P<0.05);观察组患者的护理满意度(100.00%)显著高于对照组(82.61%)(P<0.05)。结论早期综合护理可显著降低恶性肿瘤动脉化疗栓塞术后并发LEDVT的风险,并可降低肺栓塞及感染等并发症的发生率,提高患者术后生活质量,临床护理效果满意。

Objective To explore the effects of early comprehensive nursing on lower extremity deep venous thrombosis(LEDVT)after arterial chemoembolization for malignant tumors and its clinical nursing effects.Methods A total of 160 patients with malignant tumor treated by arterial chemoembolization between January 2017 and March 2019 in our department were selected as the study objects.They were divided into control group(73 cases)and observation group(87 cases)by random number table grouping method.Patients in control group received routine nursing,while those in observation group received early comprehensive nursing.The incidences of complications such as LEDVT and pulmonary embolism,scores of anxiety and depression,improvement of quality of life and nursing satisfaction were compared between the two groups.Results After nursing,the incidences of complications such as LEDVT,pulmonary embolism,abdominal infection,bleeding at puncture point were all significantly lower in observation group than in control group(P<0.05).Before nursing,there were no significant differences in the scores of HAMA,HAMD,and somatic,emotional,social and pain functions between the two groups(P<0.05).After nursing,the scores of HAMA,HAMD and pain function decreased in the two groups,and they were significantly lower in observation group than in control group(P<0.05).The scores of somatic,emotional and social functions increased in both groups after nursing,and the scores were remarkably higher in observation group than in control group(P<0.05).The observation group got higher nursing satisfaction rate than the control group(100.00%vs.82.61%,P<0.05).Conclusion Early comprehensive nursing could lower the risk of lower extremity deep venous thrombosis after arterial chemoembolization for malignant tumors,reduce the incidence of complications such as pulmonary embolism and infection,and improve the quality of life of patients after operation.Its clinical nursing effect was satisfactory.
