止痛润肠饮治疗混合痔术后便秘

Treatment of constipation in patients with mixed hemorrhoids treated by Zhitong Runchang Decoction

目的探讨中药止痛润肠饮治疗混合痔术后便秘的临床有效性和安全性。方法选择96例混合痔术后便秘患者为研究对象,通过随机数字表法分为研究组和对照组,各48例;2组术后均给予常规专科治疗,对照组遵医嘱服用乳果糖口服液,研究组服用中药止痛润肠饮,对比2组疗效及治疗后首次、第3天~第6天、第7天~第10天排便疼痛度,并用便秘症状积分量表、Bristol粪便分级法、大便次数评测治疗前后患者排便改善状况,掌握治疗期间不良反应发生情况。结果研究组治疗总有效率(93.75%,45/48)高于对照组(78.17%,38/48)(P<0.05);研究组治疗后首次、第3天~第6天排便疼痛度显著低于对照组(P<0.05),但第7天~第10天无显著性差异(P>0.05);治疗后研究组患者便秘症状积分、Bristol粪便分级评分、大便次数均均优于对照组(P<0.05);研究组不良反应率(4.17%,3/48)与对照组(6.25%,2/48)比较,无显著性差异(P>0.05)。结论止痛润肠饮治疗混合痔术后便秘疗效确切,可有效促进排便,改善便秘症状,且无严重不良反应,安全性良好。

Objective To explore the clinical efficacy and safety of traditional Chinese medicine Zhitong Runchang Decoction in the treatment of constipation after mixed hemorrhoids.Methods A total of 96 patients with postoperative constipation were included in the observational study.They were divided into experimental group and control group by random number table method,48 cases in each group.The general specialist treatment was given.The control group took the lactulose oral solution according to the doctor’s advice.The experimental group took the traditional Chinese medicine Zhitong Runchang Decoction,compared the efficacy of the two groups and the pain of defecation for the first time,the 3rd to 6th day,the 7th to the 10th day after treatment,and the symptoms of constipation.The integral scale,Bristol stool grading method,stool frequency evaluation,and the improvement of defecation in patients before and after treatment,and master the occurrence of adverse reactions during treatment.Results The total effective rate of the experimental group was 93.75%(45/48),which was higher than that of the control group(78.17%,38/48).The difference was statistically significant(P<0.05).The pain of defecation was significantly lower in the experimental group than in the control group for the first time and the 3rd to 6th day after treatment(P<0.05),but there was no significant difference at 7-10 d(P>0.05).Constipation symptom score,Bristol stool grading score,stool frequency in the experimental group after treatment all were better than the control group(P<0.05);the adverse reaction rate of the test group was 4.17%,and the control group was 6.25%,with no significant difference(P>0.05).Conclusion Zhitong Runchang Decoction is effective in treating constipation after mixed hemorrhoids.It can effectively promote bowel movements,improve symptoms of constipation,and has no serious adverse reactions.It has good safety and is worthy of clinical application.
