ICP孕妇血流动力学与炎性因子及妊娠结局的变化研究

Changes of hemodynamics and inflammatory factors in pregnant women with ICP and pregnancy outcome

目的探讨妊娠期肝内胆汁淤积症(ICP)对孕妇血流动力学与炎性因子及妊娠结局的影响。方法选取70例ICP孕妇为研究组,并选取同期在我院建档的70例健康孕妇为对照组,比较2组血流动力学指标和炎性因子水平差异以及妊娠结局。结果研究组LVEF、外周血管顺应性和每搏心输出量显著低于对照组,外周血管阻力及血液黏度显著高于对照组(P<0.05);研究组血清炎性因子及肝功能指标水平均显著高于对照组(P<0.05);研究组妊高症、胎膜早破及产后出血发生率均显著高于对照组(P<0.05),宫内窘迫、早产、低体质量儿、新生儿窒息及死亡的发生率也显著高于对照组(P<0.05)。结论ICP会严重影响孕妇血流动力学,引发机体的炎性反应,增加不良妊娠结局发生率,因此,本病一旦确诊,应及时采用有效的治疗措施,以改善妊娠结局。

Objective To investigate the influence of ICP on hemodynamics,cytokines and pregnancy outcome in pregnant women.Methods A total of 70 pregnant women with intrahepatic cholestasis of pregnancy were selected as the study group,and 70 healthy pregnant women who were filed in our hospital for the same period as the control group.The hemodynamic index levels,cytokine levels and pregnancy outcomes were compared in two groups.Results The LVEF,peripheral vascular compliance and cardiac output per stroke in the study group were significantly lower than those in the control group(P<0.05);the serum inflammatory factors and liver function indexes in the study group were significantly higher than those in the control group(P<0.05).The opregnancy incidence induced hypertension,premature rupture of membranes and postpartum hemorrhage was significantly higher in the study group than in the control group(P<0.05).The incidence of intrauterine distress,preterm birth,low birth weight,neonatal asphyxia and death was also significantly higher than the control group(P<0.05).Conclusion ICP could seriously affect the hemodynamics of pregnant women,trigger inflammatory reactions in the body,and increase the incidence of adverse pregnancy outcomes.Therefore,once diagnosed,effective treatment measures should be adopted in time to improve pregnancy outcomes.
