静脉滴注盐酸氨溴索联合红霉素对新生儿重症肺炎的疗效研究 被引量:18

摘要 目的观察静脉滴注盐酸氨溴索联合红霉素对新生儿重症肺炎的疗效。方法选择150例新生儿重症肺炎患儿,随机分为2组,各75例。对照组静脉滴注氨溴索,每次7.5 mg,每日2次;观察组联合静脉滴注红霉素,30 mg·kg^-1·d^-1,分3~4次使用。比较2组的血清炎性指标、血气分析指标和心肌酶水平。结果观察组有效率明显高于对照组(P<0.05);2组治疗后的血清IL-6、CRP和IL-10水平明显降低,IL-2水平明显升高,且观察组更为明显(P<0.05);2组治疗后的PaCO2明显降低,PaO2明显升高,且观察组更为明显(P<0.05);2组治疗后的血清CK-MB水平明显降低,且观察组更为明显(P<0.05)。结论静脉滴注盐酸氨溴索联合红霉素能明显改善新生儿重症肺炎的动脉血气,减轻心肌损害和炎性反应,安全可靠。 Objective To study the effect of intravenous ambroxol hydrochloride combined with erythromycin on neonatal severe pneumonia.Methods A total of 150 cases of patients with severe neonatal pneumonia were selected and divided into two groups randomly.The control group was given ambroxol intravenously 7.5 mg each time,twice a day,while the observation group was given erythromycin intravenously 30 mg·kg^-1·d^-1,3-4 times.The levels of serum inflammatory index,blood gas analysis index and myocardial enzymes were compared between the two groups.Results The effective rate of the observation group was significantly higher than control group(P<0.05).After treatment,the serum levels of IL-6,CRP and IL-10 in the two groups decreased significantly,and IL-2 increased significantly,especially in the observation group(P<0.05).After treatment,PaCO2 in the two groups decreased significantly,PaO2 increased significantly,especially in the observation group(P<0.05).The serum CK-MB level of the two groups decreased significantly after treatment,especially in the observation group(P<0.05).Conclusion Intravenous ambroxol hydrochloride combined with erythromycin could significantly improve the arterial blood gas of neonatal severe pneumonia,reduce myocardial damage and inflammatory reaction,safe and reliable.
