摘要
目的探讨帕金森病(Parkinson's disease,PD)患者脑白质网络拓扑属性的特异性改变。方法纳入22例PD患者和22例年龄、性别相匹配的健康志愿者(对照组)。均完成弥散张量成像(DTI)扫描,在纤维追踪技术基础上运用基于图论的方法分析两组脑白质网络拓扑属性差异;并进一步探讨PD患者拓扑属性相关参数值与临床资料之间的相关性。结果①两组脑白质网络均保留小世界属性。②PD组脑白质网络较对照组全局效率(Eglob)及聚类系数(Cp)均明显减小,最短路径长度(Lp)明显增加,差异均有统计学意义(P=0.02,P=0.037,P=0.047);而PD组与对照组脑白质网络的小世界属性值和局部效率(Eloc)差异均无统计学意义(P=0.407,P=0.059)。③PD组中最短路径长度值与病程呈正相关(P=0.026),与疾病严重程度H-Y分级、UPDRS-Ⅲ评分无相关性;全局效率、聚类系数、局部效率、小世界属性值均与疾病病程、H-Y分级、UPDRS-Ⅲ评分无明显相关性(P>0.05)。结论与健康对照者相比,PD患者脑白质网络拓扑属性改变,全脑白质网络整体连接性下降,整合和传递信息能力下降。随着病程的延长,PD患者脑白质网络拓扑属性的改变更加明显。
Objective To investigate the specific changes of topology properties of white matter network in patients with Parkinson disease(PD).Methods 22 PD patients and 22 healthy volunteers(control group)matched by age and sex were included.All subjects completed diffusion tensor imaging(DTI)scanning,and the topological attribute difference of white matter network between the two groups was analyzed by graph theory analysis method on the basis of fiber tracking technology.The correlation between the topological attributes of PD patients and the clinical data was evaluated.Results(1)Both groups of white matter networks retained small-world properties.(2)Compared with the control group,the global efficiency(Eglob)and clustering coefficient(Cp)of the white matter network in PD group were significantly reduced,but the shortest path length(Lp)was increased with significant difference(P=0.02,P=0.037,P=0.047).However,there was no significant difference in small world attribute value and local efficiency(Eloc)between PD group and control group(P=0.407,P=0.059).(3)In the PD group,the shortest path length value was positively correlated with the duration of disease(P=0.026),but not correlated with the H-Y grade of disease severity or UPDRS-Ⅲscore.There was no significant correlation between global efficiency,clustering coefficient,local efficiency and small-world attribute value and course of disease,H-Y grading and UPDRS-Ⅲscore(P>0.05).Conclusion Compared with healthy controls,the topology properties of the white matter network of PD patients are changed,the overall connectivity of the whole brain network is decreased,and the ability to integrate and transmit information is decreased.With the prolongation of the duration of disease,the topological properties of the white matter network in PD patients are changed more obviously.
