摘要
目的探讨骶2髂骨三皮质螺钉(S2AI)技术在脊柱骨盆稳定重建中的临床效果。方法选取2014年1月至2017年5月在周口市中心医院接受脊柱骨盆内固定术治疗60例,根据治疗方案分为S2AI组(n=30)和IS(髂骨螺钉)组(n=30),S2AI组给予S2AI技术治疗,IS组给予髂骨螺钉治疗,观察两组手术情况(手术时间、术中出血量等)、生活质量等。结果S2AI组和IS组手术时间、术中出血量、总引流量和住院时间比较差异无统计学意义(P>0.05);S2AI组术后1周和1年VAS评分分别为(2.52±0.84)分和(1.41±0.64)分,ODI指数分别为(7.51±1.54)%和(3.00±0.69)%,明显低于IS组(P<0.05);S2AI组术后1年SF-36量表评分为(81.30±9.81)分,明显高于IS组(P<0.05);S2AI组螺钉松动和损伤性螺钉位置不良比例均为0,明显低于IS组(P<0.05)。结论S2AI技术在脊柱骨盆稳定重建中有较好的临床效果,值得临床使用。
Objective To investigate the clinical effect of S2 alar-iliac screw(S2 AI)technique in stable pelvic spine reconstruction.Methods a total of 60 patients who underwent spinal and pelvic internal fixation in Zhoukou Central hospital from January2014 to May 2017 were selected,according to the selected treatment plan,the patients were divided into group S2 AI(n=30)and group IS(n=30),Group S2 AI was given S2 AI treatment,and group IS was treated with iliac screw,the operation time,intraoperative blood loss,quality of life and other operation conditions of the two groups were observed.Results There was no significant difference in operative time,intraoperative blood loss,total drainage volume and hospital stay between group S2 AI and group IS(P>0.05);The VAS scores at 1 week and 1 year after operation in group S2 AI were(2.52±0.84)and(1.41±0.64),the ODI index was(7.51±1.54)%and(3.00±0.69)%respectively,which were significantly lower than that in group IS(P<0.05);The score of SF-36 scale at years after operation in group S2 AI was(81.30±9.81),which was significantly higher than that in group IS(P<0.05);In group S2 AI,the ratio of screw loosening and injury screw position were 0.00%,which were significantly lower than that in group IS(P<0.05).Conclusion S2 AI technology has a good clinical effect in stable pelvic spine reconstruction,and is worthy of clinical application.
