血清肿瘤标志物及HSP90α联合检测诊断大肠癌临床价值的探讨

The clinical value of the combined detection of serum tumor markers and HSP90α in the diagnosis of colorectal cancer

目的探讨血清肿瘤标志物癌胚抗原(CEA)、糖类抗原19-9(CA19-9)、糖类抗原24-2(CA24-2)、糖类抗原72-4(CA72-4)和热休克蛋白90α(HSP90α)联合检测诊断大肠癌的意义。方法收集2016年1月-2018年12月收治的106例大肠癌患者作为大肠癌组,选取同期健康体检者46例作为对照组,检测血清中的CEA、CA19-9、CA24-2、CA72-4和H SP90α浓度。结果与对照组相比,大肠癌组5项肿瘤标志物水平均显著升高(P<0.05)。相较于TNMⅠ期~Ⅱ期患者,Ⅲ瓜期~Ⅳ期患者5项肿瘤标志物水平均显著升高(P<0.05)。CEA、CA19-9、CA24-2和HSP90α 4项联合检测诊断大肠癌灵敏度、诊断符合率均显著高于单项检测(P<0.05),相比于5项联合差异无统计学意义(P>0.05)。治疗后和治疗前相比,5项肿瘤标志物水平均显著降低(P<0.05)。结论血清CEA、CA 19-9、CA24-2及HSP90α4项肿瘤标志物联合检测可提高大肠癌诊断符合率,有助于大肠癌的早期诊断,疗效评价及预后评估。

Objective To investigate the clinical significance of combined detection of serum tumor markers,including carcinoembryonic antigen( CEA),carbohydrate antigen 199( CA19-9),carbohydrate antigen 242( CA24-2),carbohydrate antigen724( CA72-4) and heat shock protein 90 α( HSP90α),in the diagnosis of colorectal cancer. Methods A total of 106 patients with colorectal cancer admitted to the hospital from January 2016 to December 2018 were selected as the colorectal cancer group,46 healthy people in the same period were selected as the control group. CEA,CA19-9,CA24-2,CA72-4 and HSP90α levels were measured. Results Compared with the control group,the levels of five tumor markers in the colorectal cancer group were significantly increased( P < 0. 05). Compared with TNM Ⅰ-Ⅱ patients,the levels of 5 tumor markers in TNM Ⅲ-Ⅳ patients were significantly increased( P < 0. 05). The sensitivity and diagnostic coincidence rate of the combined detection of CEA,CA19-9,CA24-2 and HSP90α in the diagnosis of colorectal cancer were significantly higher than that of single detection( P < 0. 05),and the difference was not statistically significant compared with the combined detection( P >0. 05). After treatment and before treatment,the levels of all five tumor markers were significantly reduced( P < 0. 05).Conclusion The combined detection of serum CEA,CA19-9,CA24-2 and HSP90α can significantly improve the diagnostic coincidence rate of colorectal cancer. At the same time, it is helpful to the early diagnosis, prognosis judgment and therapeutic effect.
