锁骨浆细胞骨髓瘤的影像特点分析

摘要 目的总结锁骨浆细胞骨髓瘤的影像特征,提高对本病的影像认识水平。方法分析本院2013年3月~2018年12月期间11例锁骨肿瘤性病变并经手术病理诊断为浆细胞骨髓瘤的影像表现,其中X线、CT及MRI均检查6例,X线和CT检查4例,单行CT检查1例。结果X线表现膨胀性改变明显8例,膨胀性改变不明显2例,骨皮质变薄、中断6例。11例CT表现均有不同程度的膨胀性改变及骨小梁破坏、消失,其中5例病灶中骨嵴形成,8例软组织肿胀明显,伴肿块形成。6例MRI检查中有2例病灶信号混杂,边界不清,4例呈长T1长T2信号影,脂肪抑制像上条片状较T2更高信号影。结论锁骨浆细胞骨髓瘤在X线、CT及MRI分别有不同的影像特征;三种检查方法联合使用能全面展示浆细胞骨髓瘤的影像特征,更有利于提高对该病的认识。 Objective To summarize the imaging features of clavicular plasma cell myeloma and improve the understanding level of its image.Methods The imaging findings of 11 cases of clavicular tumor diagnosed as plasma cell myeloma by surgical pathology in our hospital from March 2013 to December 2018 were analyzed,including 6 cases of X-ray,CT and MRI,4 cases of X-ray and CT,and 1 case of CT alone.Results X-ray findings found that the expansion changes were obvious in 8 cases,not obvious in 2 cases,and 6 cases had bone cortex thinning or interruption.CT findings found that all 11 cases had expansion changes with different degrees and destruction and disappearance of bone trabecular,among which 5 cases had formed bone crest in the lesion,and 8 cases had obvious swelling of soft tissue accompanied with mass formation.Among the 6 cases of MRI examination,there were 2 cases with mixed signal and unclear boundary of the lesion,4 cases with long T1 and long T2 signal shadow,and the strip and plate shadow on the fat suppression image had higher signal than T2.Conclusion Clavicular plasma cell myeloma has different imaging features in the tests of X-ray,CT and MRI.The combined use of the three methods can fully display the imaging features of plasma cell myeloma,which is more conducive to improving the understanding of the disease.
