南京市妇科恶性肿瘤患者恐惧疾病进展现状及危险因素的调查研究

Status Quo and Risk Factors of Fear of Cancer Progression in Gynecological Malignant Tumors Patients in Nanjing

目的探讨南京市妇科恶性肿瘤患者恐惧疾病进展(fear of cancer progression,FCP)现状,并分析其影响因素。方法采用随机分层抽样法抽取南京市6个区12所医院1872例住院的妇科肿瘤患者,采用汉化癌症患者恐惧疾病进展简化量表(fear of progression questionnaire-short form,FoP-Q-SF)评估其FCP,并分析影响因素。结果本研究共纳入1872例患者,FoP-Q-SF评分为(36.26±6.35)分,其中≥34分有1145例(61.16%),存在恐惧疾病进展;多元线性回归分析显示,年龄、肿瘤分期、疼痛程度、睡眠障碍、病耻感是FCP的危险因素,文化程度、家庭人均月收入、社会支持是保护因素(均P<0.01)。结论妇科恶性肿瘤患者FCP处于较高水平,医务人员对不同需求患者进行个体化干预,以减轻患者心理负担,促进其康复。

Objective To explore the status quo of Fear of Cancer Progression(FCP)in patients with gynecological malignant tumors in Nanjing,and to analyze its influencing factors.Methods A random stratified sampling method was used to select a total of 1872 patients with gynecological tumors from 12 hospitals indistricts of Nanjing.The short version of the Fear of Progression Questionnaire(FoP-Q-SF)in Chinese was used to evaluate FCP,and to analyze the influencing factors of FCP in patients with gynecological malignant tumors.Results A total of 1872 cases were included in the study.The scores of FoPQSF were 36.26±6.35,and these scores of 1145 cases(61.16%)were equal to or above 34 which meant high FCP.Multiple linear regression analysis showed that age,tumor stage,pain degree,sleep disorder,and stigma were risk factors for FCP,and education level,family monthly income per capita,and social support were protective factors(all P<0.01).Conclusions The FCP of gynecological malignant tumor patients is at a high level.Medical staff can conduct individualized interventions for patients with different needs to reduce their psychological burden and promote their recovery.
