基于潜在不适当用药标准分析发生药品不良反应的老年住院患者用药情况

Analysis of medication use in elderly inpatients with adverse drug reactions based on potentially inappropriate medication standards

目的结合老年潜在不适当用药(PIM)目录,分析发生药品不良反应(ADR)的老年住院患者用药情况,为临床合理用药提供参考。方法依据2019版Beers标准和《中国老年人潜在不适当用药判断标准》(2017版)目录,将我院发生ADR大于65岁的住院患者138例,住院期间的药品进行分类,分析不适当用药情况及影响因素。结果按照Beers标准,98例(71.0%)存在PIM,合计260项。按照《中国老年人潜在不适当用药判断标准》,90例(65.2%)存在PIM,共计142项。结论在发生ADR的老年住院患者中较普遍存在不适当用药现象,临床工作中应结合2种标准,采取相应的措施降低PIM发生率,提高医院用药水平。

Objective To determine the medication use of elderly inpatients with adverse drug reactions(ADR)combined with the elderly potentially inappropriate medication(PIM)catalogue,so as to provide references for clinical rational use of drugs.Methods According to the 2019 Beers criteria and the criteria of PIM for older adults in China(2017),the medication use for 138 inpatients with ADR(≥65 years old)during hospitalization were classified,and the inappropriate medication use and influence factors were analyzed.Results According to the Beers criteria,there were 98(71.0%)cases of PIM,with a total of 260 items.According to the criteria of PIM for older adults in China,there were 90(65.2%)cases of PIM,with a total of 142 items.Conclusion Inappropriate medication is more common in elderly inpatients with ADR.The two standards should be combined in clinical settings,and corresponding measures should be taken to reduce the incidence of PIM and improve the level of medication.
