老年睡眠呼吸暂停综合征并发脑血栓形成临床分析

Clinical analysis of cerebral thrombosis in elderly patients with sleep apnea syndrome

目的:分析老年睡眠呼吸暂停综合征并发脑血栓形成的临床治疗。方法:2017年11月-2019年11月收治睡眠呼吸暂停综合征并发脑血栓形成患者40例,分为两组,各20例。对照组为32~60岁的青年患者,观察组为60~90岁的老年患者;观察两组睡眠监测结果、体检资料及凝血指标。结果:对照组微觉醒指数、氧减指数、Ⅲ期睡眠时间百分比及总睡眠呼吸暂停时间均高于观察组,Ⅰ期睡眠时间百分比低于观察组,差异有统计学意义(P<0.05);两组性别、体重、BMI及颈围比较,差异有统计学意义(P<0.05);观察组凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)指标水平低于对照组,纤维蛋白原(FIB)、D-二聚体等指标高于对照组,差异均有统计学意义(P<0.05)。结论:年龄越大,睡眠呼吸暂停综合征并发脑血栓形成的病情越严重,越需要及时有效的治疗。

Objective:To analyze cerebral thrombosis in elderly patients with sleep apnea syndrome.Methods:From November 2017 to November 2019,40 patients with sleep apnea syndrome complicated with cerebral thrombosis were selected,they were divided into the two groups with 20 cases in each group.The control group was young patients aged 32 to 60.The observation group was elderly patients aged 60 to 90.The sleep monitoring results,physical examination data and blood coagulation indexes of the two groups were observed.Results:The microarousal index,oxygen reduction index,the percentage of stage Ⅲ sleep time and total sleep apnea time in the control group were higher than those in the observation group,while the percentage of stage Ⅰ sleep time in the control group was lower than that in the observation group,the difference was statistically significant(P<0.05).There were significant differences in sex,body weight,BMI and neck circumference between the two groups(P<0.05).The levels of TT,PT and APTT in the observation group were lower than those in the control group,while the levels of D-dimer and FIB in the observation group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The older the age,the more serious the condition of sleep apnea syndrome complicated with cerebral thrombosis,which needs timely and effective treatment.
