摘要 目的分析冠心病(CAD)的影响因素。方法选取2018年1月~2019年12月石河子市人民医院收治的1086例行冠脉造影患者作为研究对象。根据世界卫生组织(WHO)的CAD诊断标准,将患者分为CAD组(666例)和非CAD组(420例)。收集所有患者的一般资料。分析两组患者的生化指标,采用多因素二元Logistic回归分析CAD的影响因素。结果单因素分析结果显示,CAD组的男性占比、年龄、吸烟占比、高血压占比、糖尿病占比、尿酸水平高于非CAD组,高密度脂蛋白胆固醇水平低于非CAD组,差异有统计学意义(P<0.05);两组患者的体重指数、总胆固醇、三酰甘油、低密度脂蛋白胆固醇比较,差异无统计学意义(P>0.05)。多因素二元Logistic回归分析结果显示,男性(β=0.452,OR=1.571,95%CI=1.048~2.356)、高龄(β=0.053,OR=1.055,95%CI=1.041~1.068)、吸烟(β=0.697,OR=2.007,95%CI=1.359~2.963)、糖尿病(β=0.618,OR=1.856,95%CI=1.352~2.548)、高尿酸(β=0.002,OR=1.002,95%CI=1.000~1.003)为CAD的危险因素(P<0.05),高密度脂蛋白胆固醇(β=-0.708,OR=0.493,95%CI=0.333~0.730)为CAD的保护因素(P<0.05)。结论高尿酸、男性、高龄、吸烟、糖尿病可能是CAD的危险因素,高密度脂蛋白胆固醇可能是CAD的保护因素。 Objective To investigate the factors influencing of coronary artery disease(CAD).Methods A total of 1086 consecutive patients who were enrolled in the Shihezi Municipal People′s Hospital from January 2018 to December 2019 were selected as subjects.In accordance with the World Health Organization(WHO)diagnosis standard of CAD,they were divided into CAD group(666 cases)and non-CAD group(420 cases).General information on all patients was collected.The biochemical characteristics were examined,and multiple factors were analyzed by Logistic regression analysis.Results Unifactor analysis showed that the proportion of male,age,the proportion of smoking,the proportion of hypertension,the proportion of diabetes and uric acid level in the CAD group were higher than those in the non-CAD group,and the high-density lipoprotein cholesterol level was lower than that in the non-CAD group,with statistically significant differences(P<0.05).The results of multivariate Logistic regression analysis showed that male(β=0.452,OR=1.571,95%CI=1.048-2.356),advanced age(β=0.053,OR=1.055,95%CI=1.041-1.068),smoking(β=0.697,OR=2.007,95%CI=1.359-2.963),diabetes(β=0.618,OR=1.856,95%CI=1.352-2.548),high uric acid(β=0.002,OR=1.002,95%CI=1.000-1.003)were the risk factors for CAD(P<0.05),high density lipoprotein cholesterol(β=-0.708,OR=0.493,95%CI=0.333-0.730)was the protection factor for CAD(P<0.05).Conclusion The present study indicates that high uric acid,being male,advanced age,smoking,and diabetes may be risk factors for CAD,while high density lipoprotein cholesterol may be a protective factor for CAD.
