卡前列素氨丁三醇联合宫腔球囊放置术在前置胎盘剖宫产产后出血中的应用效果

Application effect of Tromethamine combined with intrauterine balloon placement in postpartum hemorrhage after cesarean section of placenta previa

目的探讨卡前列素氨丁三醇联合宫腔球囊放置术在前置胎盘剖宫产产后出血中的应用效果。方法选取2015年10月~2018年12月江西省宜春市妇幼保健院收治的220例前置胎盘剖宫产产后出血患者作为研究对象,按照随机数字表法将其分为对照组与观察组,每组各110例。对照组患者采用缩宫素联合宫腔球囊放置术治疗,观察组患者采用卡前列素氨丁三醇联合宫腔球囊放置术治疗。比较两组患者的血红蛋白含量、产后出血情况及不良反应发生情况。结果术后两组患者的血红蛋白含量均低于术前,差异有统计学意义(P<0.05)。术后观察组患者的血红蛋白含量高于对照组,差异有统计学意义(P<0.05)。观察组患者产后2、24 h出血量少于对照组,差异有统计学意义(P<0.05)。两组患者恶心呕吐、一过性血压升高、发热、胃肠道反应等不良反应发生率比较,差异无统计学意义(P>0.05)。结论卡前列素氨丁三醇联合宫腔球囊放置术可降低前置胎盘剖宫产产后出血量。

Objective To investigate the application effect of Tromethamine combined with intrauterine balloon placement in postpartum hemorrhage after cesarean section of placenta previa.Methods A total of 220 patients with postpartum hemorrhage after cesarean section of placenta previa who were admitted to Yichun Maternal and Child Health Hospital of Jiangxi Province from October 2015 to December 2018 were selected as the research objects and divided into the control group and the observation group by random number table method,with 110 cases in each group.The control group was treated with Oxytocin combined with intrauterine balloon placement.The observation group was treated with Tromethamine combined with intrauterine balloon placement.The hemoglobin contents,postpartum hemorrhage and adverse reactions were compared between the two groups.Results The hemoglobin contents in the two groups after surgery were lower than those before surgery,and the differences were statistically significant(P<0.05).The hemoglobin content in the observation group after surgery was higher than that in the control group,and the difference was statistically significant(P<0.05).The blood loss in the observation group at 2 and 24 h postpartum was less than that in the control group,and the differences were statistically significant(P<0.05).There were no statistically significant differences in the incidence rates of adverse reactions such as nausea and vomiting,transient hypertension,fever,gastrointestinal reactions between the two groups(P>0.05).Conclusion Tromethamine combined with intrauterine balloon placement can reduce the amount of postpartum hemorrhage after cesarean section of placenta previa.
