三踝骨折术后中药三期辨治对踝关节活动功能的影响

Effects of Three-stage Differentiation and Treatment of TCM on Ankle Joint Movement Function after the Surgery of Cotton Fracture

目的:观察三踝骨折术后中药三期辨治对踝关节活动功能的影响。方法:将70例患者随机分为两组,每组35例。对照组实施切开复位内固定手术治疗,术后常规康复治疗;观察组在此基础上,根据中医骨伤三期辨证中药口服外洗治疗。观察治疗前后两组患者踝关节活动度、踝关节功能评分(Baird-Jackson评分).疼痛视觉模拟评分(visual analog scale,VAS)及焦虑自评量表(self-rating anxiety scale,SAS)评分的变化情况。结果:治疗后观察组跖屈、背伸、内翻、外翻等踝关节活动度改善情况优于对照组,差异有统计学意义(P<0.05)。Baird-Jackson评分治疗后观察组高于对照组,差异有统计学意义(P<0.05)。VAS及SAS评分治疗后两组比较,差异有统计学意义(P<0.05)。总有效率对照组为85.71%(30/35),观察组为97.14%(34/35),两组比较差异有统计学意义(P<0.05).结论:三踝骨折术后中药三期辨治,能促进踝关节功能恢复,减轻患者痛苦。

Objective: To observe the effects of three-stage differentiation and treatment of TCM on the function of ankle joint movement after the operation of cotton fracture. Methods: All 70 patients were randomized into two groups, 35 cases each group. The control group accepted open reduction and internal fixation, and conventional recovery after the operation;and the observation group oral administration and external wash of herbs according to three-stage differentiation and treatment of TCM orthopedics and traumatology. To observe ankle activity, Baird-Jackson scores, VAS scores and SAS scores of both groups before and after treating. Results: After treating, the improvements of ankle joint activity including plantar flexion, dorsiflexion, introversion, eversion and others in the observation group were superior to these of the control group, and the difference had statistical meaning(P<0.05). The observation group was higher than the control group in Baird-Jackson scores after treating, and the difference was statistically significant(P<0.05). The difference had no statistical meaning in VAS and SAS scores before treating between both groups(P<0.05). Total effective rate of the control group was 85.71%(30/35), lower than 97.14%(34/35) of the observation group, and the difference had statistical meaning between both groups(P <0.05). Conclusion: Three-stage differentiation and treatment of TCM could promote functional recovery of ankle joint movement and help relieve the patients′ pain after the operation of cotton fracture.
