自拟扶正愈痈汤治疗肛周脓肿术后创面愈合的临床效果观察

Clinical Effect of Self-made Fuzheng Yuyong Decoction on Wound Healing after Perianal Abscess Operation

目的:观察自拟扶正愈痈汤治疗肛周脓肿术后创面愈合的临床效果。方法:选取2018年7月-2020年4月于莆田市第一医院接受肛周脓肿手术的患者82例,通过系统随机化法分为试验组与对照组,各41例。对照组予以肛周脓肿一期根治术及常规抗感染治疗,试验组在对照组基础上采用自拟扶正愈痈汤治疗。比较两组临床疗效。结果:治疗后,试验组血清炎症因子(CRP、TNF-α、IL-6)水平均低于对照组,差异有统计学意义(P<0.05)。与对照组比较,试验组创面愈合时间更早,肉芽组织生长评分更低,差异有统计学意义(P<0.05)。术后7、15 d,试验组VAS及创面渗出情况评分均低于对照组,差异有统计学意义(P<0.05)。试验组治疗总有效率为95.12%,对照组治疗总有效率为80.49%,差异有统计学意义(P<0.05)。结论:为肛周脓肿手术的患者实施自拟扶正愈痈汤治疗可促进创面愈合,有效缓解炎症反应及创面渗出,有利于减轻患者疼痛,值得临床推广。

Objective:To observe the clinical effect of self-made Fuzheng Yuyong Decoction on wound healing after surgery for perianal abscess.Method:A selection of 82 patients who underwent perianal abscess surgery in the First Hospital of Putian City from July 2018 to April 2020 were divided into the experimental group and the control group by systematic randomization,with 41 cases in each group.The control group received one-stage radical resection of perianal abscess and routine antiinfective therapy,and the experimental group was treated with self-made Fuzheng Yuyong Decoction on the basis of the control group.Compare the clinical efficacy of the two groups.Result:After treatment,the levels of serum inflammatory factors(CRP,TNF-α,IL-6)in the experimental group were lower than those in the control group,the differences were statistically significant(P<0.05).Compared with the control group,the experimental group had earlier wound healing time and lower granulation tissue growth score,the differences were statistically significant(P<0.05).Seven and fifteen days after surgery,the VAS and wound exudation scores in the experimental group were lower than those in the control group,the differences were statistically significant(P<0.05).The total effective rate of the experimental group was 95.12%,and the total effective rate of the control group was 80.49%,the difference was statistically significant(P<0.05).Conclusion:The self-made Fuzheng Yuyong Decoction for patients undergoing perianal abscess surgery can promote wound healing,effectively alleviate inflammation and wound exudation,and help relieve pain in patients.It is worthy of clinical promotion.
