感染与强直性脊柱炎的相关性

Analysis of correlation between infectious factors and the incidence of ankylosing spondylitis

强直性脊柱炎(ankylosing spondylitis,AS)是一种血清反应阴性,以中轴关节慢性炎症为主,也可累及内脏及其他组织的慢性进展性风湿免疫性疾病。主要表现为受累中轴关节和外周关节的疼痛、强直、功能障碍等。发病与遗传、感染、内分泌失调、创伤、自身免疫功能紊乱等有关。其中,感染被认为是AS发病的一个重要的诱发因素,也能影响病情的进展。本文总结分析显示从感染部位及器,感染主要发生在呼吸系统及消化系统,眼睛也常受累;相关感染菌群主要有肺炎克雷伯菌、幽门螺杆菌、链球菌等,相关感染病毒主要有EB病毒、单纯疱疹病毒、巨细胞病毒、乙型肝炎病毒等,相关感染衣原体主要有肺炎衣原体、沙眼衣原体等。所以在AS的诊断、治疗和预防中不能忽视微生物感染的致病作用。应积极开展对感染的检测和感染原筛查,并进行相应干预,以提高AS的诊疗效果。

Ankylosing spondylitis is a chronic progressive rheumatic immune disease with negative serum reaction,mainly chronic inflammation of axial joints,and can also involve viscera and other tissues.Mainly manifested as pain,rigidity,and dysfunction in the central and peripheral joints.The pathogenesis is related to heredity,infection,endocrine disorders,trauma,and autoimmune disorders.Among,infection is considered to be an important predisposing factor for the onset of AS and it can also affect the progress of the disease.According to the comprehensive analysis of this study,it was found that from the infected site and organs,the infection mainly occurred in the respiratory system and digestive system,and the eyes are often involved.Related infections include Klebsiella pneumoniae,Helicobacter pylori,and Streptococcus.Related infections include EB virus,herpes simplex virus,cytomegalovirus,and hepatitis B virus.The main infections of Chlamydia are Chlamydia pneumoniae and Chlamydia trachomatis.Therefore,the pathogenic role of infectious factors cannot be ignored in the diagnosis,treatment and prevention of AS.Infection detection and pathogen screening should be carried out actively,and corresponding intervention should be carried out to improve the diagnosis and treatment effect of AS.
