颈源性头痛辨证分型分析

TCM Syndrome Analysis of Cervical Headache

颈源性头痛为临床最常见头痛,低头族的大量增加,致该病呈高发态势。该文从中医病因、病位、病机、证型4个方面对颈源性头痛的中医证型研究现状,进行了初步的系统分析。认为该病中医病位在"颈""项",病因为正气亏虚基础上的颈椎异常姿势,且多有外受风寒、情志不畅、外伤、劳累等诱因。病机要点为气阳两虚或肝肾亏虚,寒凝血瘀,经气不畅。中医证型可分风寒证、风湿证、瘀血证、痰湿证、气血亏虚证、阳虚证、肝肾亏虚证、肝阳上亢证等,以气阳两虚或肝肾亏虚,寒凝血瘀多见。经证可分为督脉经证、太阳经证、阳明经证、少阳经证、厥阴经证,以督脉、太阳经、少阳经证多见。应用及研究存在较多问题,如病位、病因缺乏针对性,对基本证型、主要证型缺少系统研究,无规范的基本证型,病因病机与辨证分型不一致,辨证分型与治疗不一致等,现有中医头痛不能体现本病特点。加强并规范颈源性头痛证型相关因素研究,制定基本证型,将是颈源性头痛中医研究的一个基本任务。

Cervical headache is the most common headache in clinical practice,with a large increase in bowed heads,causing a high incidence of this disease.In this paper,a preliminary systematic analysis is made on the research status of TCM syndrome types of neck-induced headache from the four aspects of TCM etiology,location of disease,pathogenesis and syndrome types.It is believed that this disease is located in the"neck"and"nape".The cause is abnormal posture of the cervical spine based on the deficiency of righteousness,and there are many inducements such as external cold,bad mood,trauma,and fatigue.The main points of the pathogenesis are deficiency of Qi and Yang,or deficiency of liver and kidney,cold coagulation and blood stasis,and poor Qi.TCM syndrome types can be divided into wind-cold syndrome,rheumatism syndrome,blood stasis syndrome,phlegm-dampness syndrome,Qi-blood deficiency syndrome,Yang-deficiency syndrome,liver-kidney deficiency syndrome,liver-Yang deficiency syndrome,etc.Deficiency,cold coagulation and stasis are more common.The classics can be divided into Dumai classics,Taiyang classics,Yangming classics,Shaoyang classics and Jueyin classics.Dumai,Taiyang classics and Shaoyang classics are more common.There are many problems in application and research,such as lack of targetedness and cause of the disease,lack of systematic research on basic syndromes and main syndromes,lack of standardized basic syndromes,inconsistent etiology,pathogenesis,and syndrome differentiation,syndrome differentiation,and treatment inconsistencies,etc.,the existing TCM headaches cannot reflect the characteristics of the disease.Strengthening and standardizing the research on related factors of necktype headache syndromes and formulating basic syndrome types will be a basic task of traditional Chinese medicine research on neck-type headaches.
