基于肌骨超声的清痹颗粒外治膝骨关节炎(湿热痹阻证)临床疗效评价

Clinical Evaluation of Qingbi Granule for External Treatment of Knee Osteoarthritis (Damp-Heat Arthralgia Syndrome) Based on Musculoskeletal Ultrasound

目的观察清痹颗粒对膝骨关节炎(KOA)的临床疗效及对关节滑膜厚度、关节积液等指标的影响。方法将64例患者随机分为治疗组和对照组,每组32例,排除脱落病例,最后治疗组30例,对照组30例纳入统计。治疗组给予清痹颗粒膝关节外敷,对照组给予双氯芬酸二乙胺乳胶剂膝关节外涂。两组患者于治疗前,治疗1、2周后采用疼痛视觉模拟评分法(VAS)评判患者的膝关节疼痛程度;同时于治疗前,治疗两周后采用西部安大略麦克马斯特大学骨关节炎指数可视化量表(WOMAC)评定患膝的关节活动度,后膝关节积液量、滑膜厚度变化,并对两组疗效进行评价。结果对照组总有效率为70%(21/30),治疗组总有效率为93.3%(28/30),且治疗组优于对照组(P<0.05);治疗1、2周后,两组VAS评分、WOMAC、膝关节积液量、滑膜厚度评分较前有明显改善(P<0.05),且治疗组优于对照组(P<0.05)。结论清痹颗粒治疗膝骨关节炎临床疗效确切,可以减轻KOA患者膝关节的疼痛症状,降低滑膜厚度,改善关节功能。

Objective To observe the clinical effect of Qingbi Granule on knee osteoarthritis(KOA) and its influence on synovial membrane thickness and joint effusion.Methods A total of 64 patients were randomly divided into treatment group and control group,32 cases in each group,excluding exfoliated cases.Finally,30 cases in the treatment group and 30 cases in the control group were included in the statistics.The treatment group was given Qingbi Granule for knee joint external application,while the control group was given diclofenac diethylamine latex for knee joint external application.Before treatment,pain visual analogue scale(VAS) was used to evaluate the degree of knee joint pain in the two groups.Before treatment,two weeks after treatment,the degree of knee joint activity and the volume of posterior knee joint effusion were assessed by the Visual Osteoarthritis Index Scale(WOMAC) of McMaster University,Ontario,Western Ontario.Thickness of synovium was changed and the curative effect of the two groups was evaluated.Results The total effective rate of the control group was 70%(21/30),the total effective rate of the treatment group was 93.3%(28/30),and the treatment group was better than the control group(P<0.05);after 1 or 2 weeks of treatment,the VAS score,WOMAC,knee joint effusion volume,synovial thickness score of the two groups were significantly improved(P<0.05),and the treatment group was better than the control group(P<0.05).Conclusion Qingbi Granule is effective in treating knee osteoarthritis.It can relieve knee pain,reduce synovial thickness and improve joint function in KOA patients.
