摘要
目的研究在颅脑损伤术中急性脑膨出中应用实时超声的效果。方法38例颅脑损伤开颅术患者,按照随机的方式分为参照组及治疗组,每组19例。参照组在术中出现急性脑膨出时,即刻给予减张缝合、包扎切口后送放射科行CT检查,治疗组在术中出现急性脑膨出时,立刻在手术台边进行实时超声检查。对比两组患者手术时间和术后6个月功能恢复情况。结果治疗组患者手术时间<5 h占比为68.42%,明显高于参照组的21.05%,差异具有统计学意义(P<0.05)。术后6个月,治疗组患者的功能恢复评分为(24.78±2.06)分,明显低于参照组的(35.26±2.41)分,差异具有统计学意义(P<0.05)。结论实时超声可以及时判断颅脑损伤术中急性脑膨出的原因,为清除颅内血肿提供导向,有效缩短手术时间,提升手术效果,改善患者预后,值得推广和应用。
Objective To study the effect of real-time ultrasound in acute encephalocele during craniocerebral injury.Methods A total of 38 patients with craniocerebral injury were randomly divided into control group and treatment group,with 19 cases in each group.In the control group,when acute encephalocele occurred during the operation,the patients were immediately given tension-reducing sutures,bandaging the incision and then sent to the radiology department for CT examination.In the treatm ent group,when acute encephalocele occurred during the operation,real-tim e ultrasound examination was performed on the operating table immediately.The operation time and functional recovery after 6 months were compared between the two groups.Results The operation time<5 h rate of the treatment group was 68.42%,which was obviously higher than 21.05%of the control group,and the difference was statistically significant(P<0.05).6 months after operation,the functional recovery score of the treatment group was(24.78±2.06)points,which was obviously lower than(35.26±2.41)points of the control group,and the difference was statistically significant(P<0.05).Conclusion Real-time ultrasound can judge the cause of acute encephalocele during craniocerebral injury in time,provide guidance for clearing intracranial hematoma,effectively shorten the operation time,improve the operation effect and improve the prognosis of patients,which is worth popularizing and applying.
