宫颈细胞学、高危型HPV病毒检测联合阴道镜在宫颈病变诊治中的效果探讨

Effect of cervical cytology, high-risk HPV virus detection combined with colposcopy in the diagnosis and treatment of cervical lesions

目的探讨宫颈细胞学、高危型人乳头瘤病毒(HPV)检测联合阴道镜检测在宫颈病变中的诊断价值。方法选取500例行宫颈筛查者作为研究对象,均进行宫颈细胞学检测、高危型HPV病毒检测及宫颈细胞学检测+高危型HPV病毒检测+阴道镜检测,以病理学检测结果为金标准,比较三种检测方式的诊断符合率。结果高危型HPV病毒检测、宫颈细胞学检测及高危型HPV病毒检测+宫颈细胞学检测+阴道镜检测对鳞癌的检出例数均为13例,检出率均为100.00%。高危型HPV病毒检测+宫颈细胞学检测+阴道镜检测的诊断符合率为99.60%,显著高于宫颈细胞学检测的89.00%及高危型HPV病毒检测的82.40%,差异有统计学意义(P<0.05);宫颈细胞学检测的诊断符合率高于高危型HPV病毒检测,差异有统计学意义(P<0.05)。结论宫颈细胞学、高危型HPV病毒检测联合阴道镜检测能够有效提高宫颈病变的诊断符合率,减少宫颈的漏诊,使患者能够尽早接受治疗,预防宫颈癌的发生。

Objective To discuss the diagnostic value of cervical cytology,high-risk human papillomavirus virus(HPV)virus detection combined with colposcopy in the diagnosis and treatment of cervical lesions.Methods 500 patients undergoing cervical screening were selected as the research subjects.All patients were tested for cervical cytology,high-risk HPV virus,high-risk HPV virus+cervical cytology+colposcopy.With pathological test results as the gold standard,the diagnostic coincidence rates of the three test methods were compared.Results Cervical cytology,high-risk HPV virus,high-risk HPV virus+cervical cytology+colposcopy all detected 13 cases of squamous cell carcinoma,with a detection rate of 100.00%.The diagnostic coincidence rate of high-risk HPV+cervical cytology+colposcopy was 99.60%,which was significantly higher than 89.00%of cervical cytology test and 82.40%of high-risk HPV virus,and the difference was statistically significant(P<0.05).The diagnostic coincidence rate of cervical cytology test was higher than that of high-risk HPV virus test,and the difference was statistically significant(P<0.05).Conclusion Cervical cytology,high-risk HPV virus combined with colposcopy detection can effectively improve the diagnostic coincidence rate of cervical lesions,reduce missed diagnosis,enable patients to receive treatment as soon as possible,and prevent cervical cancer.
