腹腔镜完整结肠系膜切除术治疗横结肠癌的疗效

Evaluation of the effect of laparoscopic complete mesenterectomy for transverse colon cancer

目的探讨腹腔镜完整结肠系膜切除术治疗横结肠癌的疗效。方法依据治疗方式将86例横结肠癌患者分为对照组和研究组,每组43例,对照组患者接受开腹横结肠癌根治术治疗,研究组患者接受腹腔镜完整横结肠系膜切除术治疗。比较两组患者的临床疗效、一般临床指标及术后并发症发生情况。结果研究组患者的治疗总有效率为93.02%(40/43),高于对照组患者的72.09%(31/43),差异有统计学意义(P<0.05)。两组患者的淋巴结获检数比较,差异无统计学意义(P> 0.05),研究组患者的手术时间长于对照组患者,首次肛门排气时间、住院时间均短于对照组患者,术中出血量低于对照组患者,差异均有统计学意义(P<0.05)。研究组患者的术后并发症总发生率为4.65%,低于对照组患者的37.21%,差异有统计学意义(P<0.05)。结论腹腔镜完整结肠系膜切除术治疗横结肠癌效果满意,可减少并发症的发生风险。

Objective To evaluate the effect of laparoscopic complete mesenterectomy in the treatment of transverse colon cancer.Method A total of 86 patients with transverse colon cancer were selected and divided into control group and study group with 43 cases in each group according to the treatment regimen.The control group received open radical resection of transverse colon cancer,and the study group received laparoscopic transverse colectomy and extended mesenterectomy.Result The total complete response of treatment in the study group was 93.02%(40/43),which was higher than 72.09%(31/43)in the control group,the difference was statistically significant(P<0.05).There was no significant difference in the number of lymph nodes detected between the two groups of patients(P>0.05).The operation time of the study group was longer than that of the control group,and the first anal exhaust time and in hospital time were shorter than those of the control group,and the intraoperative bleeding volume was lower than that of the control group,and the difference was statistically significant(P<0.05).The total incidence of postoperative adverse events in the study group was 4.65%,which was lower than 37.21%in the control group,and the difference was statistically significant(P<0.05).Conclusion laparoscopic transverse colectomy and extended mesenterectomy is satisfactory in the treatment of transverse colon cancer and can reduce the risk of complications.
