新冠病毒肺炎在广州地区非定点医院中的临床特点

Clinical practice of handling COVID-19 cases in non-designated hospitals in Guangzhou

目的对广州地区某非定点医院筛查及诊治新型冠状病毒肺炎(COVID-19)患者的临床工作经验进行阶段性总结。方法采用标准化发热病人观察表监测并记录发热患者的病情,对数据进行统计学分析。结果 2020年1月23日至2月19日,共839例患者至中国人民解放军南部战区总医院发热门诊就诊,22例患者符合疑似患者标准被收入发热隔离病房,男性5人,女性17人,疑似病例排查时间为(1.69±0.76)h。其中3例患者CT结果显示肺组织多发磨玻璃样影并且COVID-19核酸检测结果为阳性而确诊感染,确诊时间为(33.00±6.88)h,确诊后即刻转诊定点医院进行治疗。其余19例患者间隔24 h连续2次核酸检测为阴性排除COVID-19感染,在达到出院指标后出院。其余的817例患者则建议居家或酒店隔离14 d,所有随访患者预后均良好。结论早期隔离所有疑似患者是控制疫情发展的关键,也是减少发生院内交叉感染的重要原因。

Objective To summarize the experience of diagnosis and treatment of the suspected corona virus disease 2019(COVID-19) patients in a non-designated hospital.Methods A standardized observation table for the fever patients was used.The conditions of fever patients were observed and recorded.The data was summarized for further analysis.Results A total of 839 patients were visited our fever clinic from Jan.23,2020 to Feb.19,2020.Among them,22 patients were admitted to fever isolation ward as suspected patients.The average time for checking suspected cases was(1.69±0.76)hours.Three of them were diagnosed as infected with SARA-CoV2 by PR-PCR and CT results.The average time for diagnosis was(33.00±6.88) hours.And they were immediately transferred to designated hospitals once diagnosed.The rest of 19 patients were excluded from SARA-CoV2 infection and discharged until meeting the criteria.The other 817 patients were recommended to be isolated at home or in hotel for at least 14 days.All patients presented with favorable prognosis.Conclusion Early isolation of all suspected patients was the key to control the development of the epidemic,and also played an important role to reduce the incidence of nosocomial infection.
