皮肤镜观察腋毛癣一例

A case of trichomycosis axillaris observed by dermoscope

30岁男性患者,双腋窝多汗10余年,发现腋毛黄色黏着物伴轻微臭味3个月。皮肤科情况:双侧腋毛干燥、粗糙、无光泽,部分腋毛根部附着鞘状黄白色黏着性物质,触之软不易剥离。皮肤镜检查:双侧腋毛部分毛干靠近毛根部分有黄白色半透明胶冻状鞘状物包绕,少许毛干有结节状凸起。细菌涂片查见革兰染色阳性杆菌。诊断:腋毛癣。给予罗红霉素胶囊口服、夫西地酸乳膏外用治疗后治愈。

A 30-year-old male patient presented with hyperhidrosis in both armpits for more than 10 years,the condition gradually aggravated with yellow adhesive substances of armpit hairs and slight odor for 3 months.Dermatological examination:bilateral axillary hairs were dry,rough and lusterless,and some of the axillary hair roots were attached with yellow-white sheath-like adhesive substances,which were touched softly and difficult to peel off.Dermoscopy examination:there were yellow-white translucent gel sheaths around the hair shafts near the hair roots,and a few hair shafts had nodular protrusions.Gram positive bacteria were found on bacterial smear.Diagnosis:trichomycosis axillaris.After 1 month treatment by oral administration of roxithromycin capsules combined with topical application of fusidic acid cream,the patient was cured.
