摘要
目的探究无创呼吸机在慢阻肺呼吸衰竭合并心功能不全治疗中的临床效果。方法选择该院2017年10月—2019年12月收治的慢阻肺呼吸衰竭合并心功能不全患者90例,按随机数字表法分为两组各45例。对照组行常规治疗,研究组在其基础上给予无创呼吸机治疗。比较两组治疗总有效率、治疗前后血气指标及心功能指标。结果研究组治疗总有效率为93.33%高于对照组的71.11%,差异有统计学意义(χ^2=7.601,P<0.05);研究组治疗后PaO2(77.62±6.03)mmHg、SpO2(96.53±2.95)mmHg均高于对照组(60.49±6.34)、(84.60±3.19)mmHg,研究组PaCO2(57.16±7.98)mmHg低于对照组(79.94±3.75)mmHg,差异有统计学意义(t=13.133、18.419、17.331,P<0.05);研究组LVEDD(51.06±2.17)mm、LVEDS(36.60±2.47)mm小于对照组(53.20±2.68)、(41.69±2.16)mm,LVEF(48.56±3.88)%大于对照组(42.25±3.70)%,差异有统计学意义(t=4.163、10.406、7.895,P<0.05);两组pH值比较,差异无统计学意义(P>0.05)。结论无创呼吸机在慢阻肺呼吸衰竭合并心功能不全治疗中可有效改善血气分析及心功能各指标水平,疗效确切。
Objective To explore the clinical effect of non-invasive ventilator in the treatment of chronic obstructive pulmonary respiratory failure with cardiac insufficiency.Methods A total of 90 patients with chronic obstructive pulmonary respiratory failure and cardiac insufficiency who were admitted to the hospital from October 2017 to December 2019 were selected and divided into two groups of 45 cases according to the random number table method.The control group received conventional treatment,and the study group received non-invasive ventilator treatment on the basis of it.The total effective rate of treatment,blood gas index and heart function index before and after treatment were compared between the two groups.Results The total effective rate of treatment in the study group was 93.33%,which was higher than 71.11%in the control group,and the difference was statistically significant(χ^2=7.601,P<0.05);after treatment in the study group,PaO2(77.62±6.03)mmHg,SpO2(96.53±2.95)mmHg was higher than the control group(60.49±6.34)mmHg,(84.60±3.19)mmHg,the study group PaCO2(57.16±7.98)mmHg was lower than the control group(79.94±3.75)mmHg,the difference was statistically significant(t=13.133,18.419,17.331,P<0.05);the study group LVEDD(51.06±2.17)mm,LVEDS(36.60±2.47)mm is smaller than the control group(53.20±2.68)mm,(41.69±2.16)mm,LVEF(48.56±3.88)%is greater than the control group(42.25±3.70)%,the difference was statistically significant(t=4.163,10.406,7.895,P<0.05);the difference in pH between the two groups was not statistically significant(P>0.05)Conclusion The non-invasive ventilator can effectively improve blood gas analysis and the levels of various indicators of cardiac function in the treatment of chronic obstructive pulmonary respiratory failure combined with cardiac insufficiency,and the effect is definite.
