2型糖尿病患者发生糖尿病周围神经病变的影响因素分析

Analysis of Influencing Factors of Diabetic Peripheral Neuropathy in Patients with Type 2 Diabetes

目的探讨2型糖尿病(T2DM)患者发生糖尿病周围神经病变(DPN)的影响因素。方法选择2018年2月—2020年5月该院收治的80例T2DM患者,按是否发生DPN分为两组,32例合并DPN患者设为观察组,48例未合并DPN设为对照组,分析T2DM患者发生DPN的影响因素。结果观察组糖尿病病程、体重指数(BMI)、尿酸(UA)、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、合并外周动脉病变比例数值分别为(9.90±3.54)年、(25.76±3.65)kg/m^2、(9.35±2.07)mmol/L、(8.86±1.43)%、28.13%,高于对照组的(5.26±1.02)年、(22.01±2.85)kg/m^2、(7.64±1.72)mmol/L、(7.85±1.74)%、8.33%,差异有统计学意义(t=8.586、5.148、4.013、2.725,χ^2=5.526,P<0.05);两组性别、年龄、舒张压、收缩压、胰岛素、甘油三酯(TG)、总胆固醇(TC)比较,差异无统计学意义(P>0.05);Logistic回归分析显示:合并外周动脉病变、糖尿病病程、BMI、UA、FPG、HbA1c是T2DM患者发生DPN的影响因素(P<0.05且OR≥1)。结论T2DM患者发生DPN与糖尿病病程、BMI、UA、FPG、HbA1c与合并外周动脉病变等多种因素密切相关,临床需综合治疗,以减少DPN发生。

Objective To explore the influencing factors of diabetic peripheral neuropathy(DPN)in patients with type 2 diabetes(T2DM).Methods Eighty patients with T2DM admitted to the hospital from February 2018 to May 2020 were divided into two groups according to whether DPN occurred.32 patients with DPN were set as the observation group,and 48 patients without DPN were set as the control group.Analysed influencing factors of DPN in patients with T2DM.Results The duration of diabetes,body mass index(BMI),uric acid(UA),fasting blood glucose(FPG),glycosylated hemoglobin(HbA1c),and the proportion of peripheral artery disease in the observation group were(9.90±3.54)years and(25.76±3.65)kg/m^2,(9.35±2.07)mmol/L,(8.86±1.43)%,28.13%,higher than the control group(5.26±1.02)years,(22.01±2.85)kg/m^2,(7.64±1.72)mmol/L,(7.85±1.74)%,8.33%,the difference was statistically significant(t=8.586,5.148,4.013,2.725,χ^2=5.526,P<0.05);there was not statistically significant difference in gender,age,diastolic blood pressure,systolic blood pressure,insulin,triglyceride(TG),total cholesterol(TC)(P>0.05);Logistic regression analysis showed:combined peripheral arterial disease,diabetes course,BMI,UA,FPG,HbA1c were the influencing factors of DPN in patients with T2DM(P<0.05 and OR≥1).Conclusion The occurrence of DPN in patients with T2DM is closely related to the course of diabetes,BMI,UA,FPG,HbA1c,and peripheral arterial disease.Comprehensive treatment is needed to reduce the occurrence of DPN.
