探讨高通量透析对尿毒症血液透析患者微炎症及营养不良的影响 被引量:2

摘要 目的探讨高通量透析对尿毒症血液透析患者微炎症及营养不良的影响。方法采用分组对照法,将方便选取2018年9月—2019年10月间在北京市房山区第一医院治疗的50例尿毒症血液透析患者作为研究对象,依照不同的透析方式分为常规、研究两组,其中常规组给予常规血液透析治疗,研究组为高通量透析治疗,比较两组患者的微炎症及营养参数变化、生活质量情况。结果与常规组相比,研究组患者炎症指标均有所下降,其中hs-CRP降至(4.49±1.88)mg/L,TNF-α为(30.71±6.32)ng/L,IL-6为(18.62±4.46)ng/L,差异有统计学意义(t=2.990、4.780、3.810,P<0.05);其营养指标Hb升至(75.33±4.32)g/L,TP为(69.27±3.04)g/L,Alb为(37.29±0.34)g/L,TRF为(3.28±0.15)g/L,均高于常规组,差异有统计学意义(t=6.690、12.210、18.150、19.920,P<0.05);其生活质量各项参数评分也要优于常规组,差异有统计学意义(P<0.05)。结论对尿毒症血液透析患者给予高通量透析治疗,能有效降低hs-CRP、TNF-α等炎症相关参数值,改善患者营养状况,提高治疗效果。 Objective To explore the effect of high-throughput dialysis on microinflammation and malnutrition in uremia hemodialysis patients.Methods Using the grouped control method,50 uremia hemodialysis patients who were treated in the hospital from September 2018 to October 2019 were conveniently selected as the research objects,and they were divided into two groups according to different dialysis methods.The group was given conventional hemodialysis treatment,and the study group was treated with high-throughput dialysis.The microinflammation,nutritional parameters and quality of life of the two groups of patients were compared.Results Compared with the conventional group,the inflammation indicators of the study group all decreased,of which hs-CRP decreased to(4.49±1.88)mg/L,TNF-αwas(30.71±6.32)ng/L,IL-6 was(18.62±4.46)ng/L,the difference was statistically significant(t=2.990,4.780,3.810,P<0.05);its nutritional index Hb rose to(75.33±4.32)g/L,TP was(69.27±3.04)g/L,Alb was(37.29±0.34)g/L,TRF was(3.28±0.15)g/L,which were higher than the conventional group,the difference was statistically significant(t=6.690,12.210,18.150,19.920,P<0.05).The scores of the quality of life parameters were also better than those of the conventional group,the difference was statistically significant(P<0.05).Conclusion High-throughput dialysis treatment for uremia hemodialysis patients can effectively reduce the values of inflammation-related parameters such as hs-CRP and TNF-α,improve the nutritional status of patients,improve the treatment effect,and provide new ideas for disease treatment.
