单纯小梁切除术治疗青光眼的临床效果探析

Analysis of the Clinical Effect of Simple Trabeculectomy in the Treatment of Glaucoma

目的探析单纯小梁切除术治疗青光眼患者的临床效果。方法简单随机选该院2012年12月—2019年12月收治的青光眼患者40例,以摸球法将患者均分为对照组(20例)与观察组(20例),前者为单纯小梁切除术治疗,后者为复合式小梁切除术治疗。对比两组临床疗效、功能性滤过泡形成、并发症及治疗前后的患眼眼压变化情况。结果观察组患者总有效率90.00%较对照组45.00%高,差异有统计学意义(χ^2=9.231,P=0.002<0.05);观察组患者功能性滤泡形成率85.00%较对照组50.00%高,差异有统计学意义(χ^2=5.584,P=0.018<0.05);观察组和对照组的不良反应发生率分别为20.00%、60.00%,差异有统计学意义(χ^2=6.667,P=0.010<0.05);两组患者手术前后与随访期的患眼眼压情况,观察组分别为(36.53±4.26)、(15.36±2.41)mmHg,对照组分别为(37.65±6.35)、(21.63±3.24)mmHg,两组治疗前比较差异无统计学意义(t=0.655,P>0.05),治疗后比较差异有统计学意义(t=6.944,P<0.05)。结论单纯小梁切除术虽是临床常用的青光眼治疗手术,但与复合式小梁切除术治疗相比,临床效果相对较差一些,复合式小梁切除术可有效控制患者眼压与功能性滤过泡形成,减少并发症。因此临床青光眼患者治疗,应用复合式小梁切除术治疗,更加合适。

Objective To explore the clinical effect of simple trabeculectomy in the treatment of glaucoma patients.Methods A total of 40 glaucoma patients admitted to the hospital from December 2012 to December 2019 were randomly selected.The patients were divided into a control group(20 cases)and an observation group(20 cases)by the ball method.The former adopted simple trabeculectomy treatment,the latter adopted compound trabeculectomy treatment.Comparison of clinical efficacy,functional filtering bleb formation,complications and changes in intraocular pressure before and after treatment between the two groups.Results The total effective rate of the observation group was 90.00%higher than that of the control group 45.00%.The difference was statistically significant(χ^2=9.231,P=0.002<0.05);the observation group had functional follicle formation rate of 85.00%,higher than that of the control group 50.00%,and the difference was obvious and statistically significant(χ^2=5.584,P=0.018<0.05);the adverse reaction rates of the two groups were 20.00%and 60.00%respectively,with statistics differences(χ^2=6.667,P=0.010<0.05);the intraocular pressure of the two groups of patients before,after and during the follow-up period,the observation group was(36.53±4.26)mmHg,(15.36±2.41)mmHg,control group of(37.65±6.35)mmHg and(21.63±3.24)mmHg respectively.There was no statistically significant difference between the two groups before treatment(t=0.655,P>0.05),and the difference was statistically significant after treatment(t=6.944,P<0.05).Conclusion Although simple trabeculectomy is a commonly used procedure for the treatment of glaucoma,its clinical effect is relatively poor compared with compound trabeculectomy.Compound trabeculectomy can effectively control the patient’s intraocular pressure and the formation of functional filtering blebs,reduce complications.Therefore,the application of compound trabeculectomy is more suitable for the treatment of clinical glaucoma patients.
