肛周任意皮瓣游离成形术联合肛门皮下组织美兰注射神经阻滞术治疗肛周瘙痒症30例

Treatment of 30 Cases of Perianal Pruritus by Free Anal Flap Freeplasty Combined with Anal Subcutaneous Tissue Melanin Injection Nerve Block

目的分析30例肛周瘙痒症患者给予肛周任意皮瓣游离成形术联合肛门皮下组织美兰注射神经阻滞术的疗效。方法2018年1月—2019年12月为选取时段,以此时间内方便选取该院收治的60例肛周瘙痒患者为研究对象,采取随机法,将其分为对照组,行单纯予肛门皮下组织美兰注射神经阻滞术,治疗组行肛周任意皮瓣游离成形术联合肛门皮下组织美兰注射神经阻滞术,对比两组效果。结果治疗总有效率方面,治疗组为96.67%,高于对照组的86.67%,差异有统计学意义(χ^2=5.135,P=0.032);在瘙痒程度、渗出及皮损等症状评分变化方面,治疗后3、7 d,治疗组各症状评分均明显低于对照组(P<0.05),治疗15 d后,两组症状评分差异无统计学意义(P>0.05)。结论给予肛周瘙痒患者,肛周任意皮瓣游离成形术联合肛门皮下组织美兰注射神经阻滞术,对患者瘙痒症状及相关体征能尽早缓解,治疗效果显著,手术安全可靠。

Objective To analyze the curative effect of 30 patients with perianal pruritus who were given free perianal flaps combined with anal subcutaneous tissue melanin injection.Methods From January 2018 to December 2019 as the selected time period,selected 60 patients of the hospital with perianal pruritus admitted during this period were taken as research objects,and they were randomly divided into control group,and they were injected with melanin into the subcutaneous tissue of the anus nerve block surgery,the treatment group underwent perianal arbitrary flap free plasty combined with anal subcutaneous tissue melanin injection nerve block to compare the effects of the two groups.Results In terms of the total effective rate of treatment,the treatment group was 96.67%,which was higher than the control group's 86.67%,the difference was statistically significant(χ^2=5.135,P=0.032);in terms of pruritus,exudation and skin lesions and other symptoms changes,3 and 7 days after treatment,the symptom scores of the treatment group were significantly lower than those of the control group(P<0.05).After 15 d of treatment,there was no statistically significant difference in symptom scores between the two groups(P>0.05).Conclusion For patients with perianal pruritus,free perianal flap freeplasty combined with anal subcutaneous tissue injection nerve block surgery can relieve the pruritus symptoms and related signs as soon as possible,the treatment effect is significant,the operation is safe and reliable.
