地佐辛联合舒芬太尼在腹腔镜胆囊切除术后应用对患者VAS评分、炎症反应的影响

Effects of Dezocine and Sufentanil on VAS Score and Inflammatory Response after Laparoscopic Cholecystectomy

目的探讨地佐辛联合舒芬太尼在腹腔镜胆囊切除术后的应用对患者疼痛和炎症反应的影响。方法从该院2015年4月—2019年12月期间就诊的腹腔镜胆囊切除术患者中随机选择114例,分为两组,每组57例,设置为观察组和对照组,观察组于术后予以地佐辛联合舒芬太尼,对照组仅予以舒芬太尼,比较两组治疗后不同时间段的疼痛视觉模拟评分(VAS评分)和炎性因子水平。结果术后1 h两组患者VSA评分差异无统计学意义(t=0.137,P=0.891),观察组术后6 h、12 h、24 h和48 h的VAS评分分别为(4.40±1.11)分、(3.98±1.28)分、(3.47±1.36)分、(2.09±1.12)分,对照组分别为(5.94±1.33)分、(5.59±1.16)分、(5.22±1.43)分、(4.64±1.45)分,观察组术后6 h、12 h、24 h、48 h VAS评分均低于对照组,差异有统计学意义(t=6.712、7.037、6.695、10.508,P<0.05);术前两组患者血清IL-6和IL-10水平差异无统计学意义(t=0.693、0.183,P>0.05)。术后24 h观察组的IL-6和IL-10水平为(99.44±18.26)pg/mL、(32.07±4.25)pg/mL,对照组为(120.84±21.37)pg/mL、(43.52±5.06)pg/mL,观察组均显著低于对照组,差异有统计学意义(t=5.748、13.082,P<0.05)。结论地佐辛联合舒芬太尼在腹腔镜胆囊切除术后应用临床效果较好,明显减轻患者术后疼痛感,降低炎性反应,促进预后。

Objective To investigate the effects of dezocine combined with sufentanil on patients'pain and inflammation after laparoscopic cholecystectomy.Methods 114 patient with laparoscopic cholecystectomy who visited the hospital from April 2015 to December 2019 were randomly selected and divided into two groups,with 57 cases in each group,which were set as observation group and control group.The observation group received dezocine combined with sufentanil after surgery,and the control group received only sufentanil.The visual analogue scale(VAS score)and inflammatory factor levels of the two groups were compared at different time periods after treatment.Results There was no significant difference in VSA scores between the two groups at 1 h after surgery,the difference was not statistically significant(t=0.137,P=0.891).The VAS scores of the observation group at 6 h,12 h,24 h,and 48 h after surgery were(4.40±1.11)points and(3.98±1.28)points,(3.47±1.36)points,(2.09±1.12)points,the control group was(5.94±1.33)points,(5.59±1.16)points,(5.22±1.43)points,(4.64±1.45)points,respectively the VAS scores of the observation group at 6 h,12 h,24 h and 48 h after surgery were lower than those of the control group,and the difference was statistically significant(t=6.712,7.037,6.695,10.508,P<0.05);There was no statistically significant difference in serum il-6 and il-10 levels between the two groups before surgery(t=0.693,0.183,P>0.05).The levels of IL-6 and IL-10 in the observation group were(99.44±18.26)pg/mL and(32.07±4.25)pg/mL at 24h postoperatively,and the control group were(120.84±21.37)pg/mL and(43.52±5.06)pg/mL respectively,the observation group was significantly lower than the control group,the difference was statistically significant(t=5.748,13.082,P<0.05).Conclusion Dezocine combined with sufentanil has a good clinical effect after laparoscopic cholecystectomy,which can significantly reduce postoperative pain,reduce inflammatory reactions,and promote prognosis.
