基于信息化系统的多潘立酮片安全合理用药分析

Analysis of Safe and Rational Use of Domperidone Tablets Based on Information System

目的:调查我中心多潘立酮片门诊用药现状,基于信息技术和人工智能手段,利用药品说明书中用药知识,评价不同年龄段患者的多潘立酮片处方用量情况,为临床安全、合理用药提供参考依据和用药数据。方法:收集2019年3~8月我中心应用多潘立酮片的处方,分析各年龄段患者的分布情况、处方用量情况及处方金额、用药频度(DDDs)、药物利用指数(DUI)及日均费用情况,参照多潘立酮片修订版说明书,借助智能系统的警戒作用,针对给药频次、给药剂量、日用药量、总用量、使用天数进行评价。结果:门诊使用多潘立酮片的处方共计549张,其中男性患者与女性患者分别为217例与332例;年龄26~98岁,平均年龄(71.76±12.39)岁。以61~90岁的老年患者为主[78.87%(433/549)],其中61~70岁患者处方数居第一[30.42%(167/549)]。排序前3位的年龄段依次为女性61~70岁、71~80岁、81~90岁。按说明书推荐的剂量为口服每次10 mg,每日3次,各年龄段日均剂量均为30 mg。结论:我中心门诊多潘立酮片的用药剂量符合说明书的推荐,DUI均为1.00。由于社区医院门诊老年患者居多,利用智能信息系统能为各年龄段患者提供个性化服务,有助于多潘立酮片用量的合理性。

Objective:To investigate the current situation of outpatient use of domperidone tablets in our center,based on information technology and artificial intelligence to mine and utilize the knowledge of drug instructions,and by evaluating the dosage of patients in different age groups,so as to provide reference and data for clinical safe and rational drug use.Methods:By collecting the prescriptions of domperidone in our center from March to August in 2019,we analyzed the distribution of all ages groups,prescription dosage and prescription amount,frequency of drug use(DDDs),drug utilization index(DUI)and average daily cost.With reference to the revised instruction of domperidone tablets and the warning function of the intelligent system,the dosing frequency,dosage,daily dosage,total dosage and using days were evaluated.Results:There were 549 prescriptions for domperidone tablets in the outpatient department,including 217 prescriptions for males and 332 prescriptions for females.The average age was(71.76±12.39)years old.The elderly patients aged from 61 to 90 years old were the majority[78.87%(433/549)],and the elderly patients aged from 61 to 70 years old ranked first[30.42%(167/549)].The top three age groups in the DDDs sequence were 61-70 years old females,71-80 years old females,and 81-90 years old females.The recommended dose according to the drug instruction is 10 mg each oral dose,3 times/d,and the average daily dose for all age groups is 30 mg.Conclusion:The dosing of domperidone tablets in the outpatient department of our center conforms to the recommendation of the instructions,and the DUI is all 1.00.Since most of the elderly patients in community hospitals are outpatients,the intelligent information system can provide personalized services for patients of all ages,which is conducive to the rationality of domperidone dosage.
