摘要
目的应用超声心动图左室压力-应变环评价高血压患者心肌做功的变化。方法选取2020年1月至6月在我院就诊的高血压患者70例和同期健康志愿者34名(对照组)进行研究,70例高血压患者左室内径和射血分数(EF)正常,根据血压分成2组:HTN 1组(收缩压140~159 mmHg)35例、HTN 2组(收缩压>160 mmHg)35例。应用二维斑点追踪技术得出左心室心肌整体纵向应变(GLS)和心肌整体做功指数(GWI)、整体有用功(GCW)、整体无用功(GWW)和整体做功效率(GWE),比较各组间GLS、心肌做功(GWI、GCW、GWW、GWE)的差异。结果与对照组相比,HTN 1组和HTN 2组的IVDd、PWd、LVmass稍增高,但是差异无统计学意义。与对照组比较,随着血压升高,HTN 1组和HTN 2组的GWI、GCW、GWW增高,HTN2组明显(P<0.05),GLS和GWE无明显变化(P>0.05)。结论左室压力-应变环是一种很有潜力的新技术,可更好地了解不同负荷条件下的心肌做功,为高血压患者左室收缩功能的定量评估提供了一种新方法。
Objective To quantitatively evaluate the changes of left ventricular myocardialwork indices by pressure-strain loops(PSL)using echocardiography in hapertension(HTN)patients.Methods Seventy patients with hypertension were divided into 2 groups according to the blood pressure(BP):HTN 1 group and HTN 2 group.Control group were thirty-four healthy volunteers.Use the off-line EchoPAC software to obtain left ventricular global longitudinal strain(GLS),global MW index(GWI),global constructive work(GCW),global wasted work(GWW)and MW efficiency(GWE).The differences of GLS and MW parameters were compared between each groups.Results Compared with the control group,with the increase of blood pressure,the GWI、GCW、GWW of HTN 1 group and HTN 2 group increased significantly(P<0.05),while there was no significant change in GLS and GWE(P>0.05).Conclusion PSL can quantitatively evaluate left ventricular myocardial work and provide a new method for the evaluation of left ventricular systolic function in hypertension patients.
