慢性心力衰竭患者血清胱抑素C、nt-proBNP、总胆红素的检测价值分析

Detection Value of Serum Cystatin C,nt-proBNP and Total Bilirubin in Patients with Chronic Heart Failure

目的:分析慢性心力衰竭(CHF)患者血清胱抑素C(CysC)、N末端B型脑钠肽前体(nt-proBNP)、总胆红素的检测价值。方法:选取2018年9月~2019年10月在某院进行治疗的CHF患者89例为研究对象纳入研究组,另选取36例健康体检者纳入对照组,比较两组受试者血清胱抑素C、nt-proBNP、总胆红素的水平,探讨以上三种指标在CHF患者中的检测价值。结果:分析发现研究组CysC、nt-proBNP、总胆红素均显著高于对照组(P<0.05);心功能Ⅱ级患者CysC、nt-proBNP、总胆红素均显著低于心功能Ⅲ级、Ⅳ级患者(P<0.05),且心功能Ⅲ级患者以上指标均显著低于心功能Ⅳ级患者(P<0.05)。经随访发现,在住院期间研究组89例患者存活66例(存活组),死亡23例(死亡组),且存活组患者血清CysC、nt-proBNP、总胆红素均显著低于死亡组(P<0.05)。经相关性分析发现,CysC、nt-proBNP、总胆红素、心功能分级与CHF患者不良预后呈正相关。结论:临床可检测CysC、nt-proBNP、总胆红素指标变化情况对CHF患者心功能分期进行有效判断,还可作为评估CHF患者预后的有效指标,为临床疾病发展、治疗及预后提供有效评估依据。

Objective:To analyze the detection value of serum cystatin C(CysC),N-terminal pro brain natriuretic peptide(nt-proBNP)and total bilirubin(TBIL)in patients with chronic heart failure(CHF).Methods:A total of 89 CHF patients who were treated in a hospital from September 2018 to October 2019 were enrolled as study group,and another 36 healthy check-ups were included in the control group,then the levels of serum CysC,nt-proBNP and TBIL were compared between the two groups,and their detection value for CHF patients was explored.Results:The levels of serum CysC,nt-proBNP and TBIL in study group were significantly higher than those in control group(P<0.05).The levels of serum CysC,nt-proBNP and TBIL in patients with cardiac function at grade II were significantly lower than those at grade III and IV(P<0.05),and the above indexes of patients with cardiac function at grade III were significantly lower than those of patients with cardiac function at grade IV(P<0.05).It was found by follow-up that during hospitalization,there were 66 cases survived(survival group)and 23 cases died(death group)in study group.Serum CysC,nt-proBNP and total bilirubin in the survival group were significantly lower than those in the death group(P<0.05).Correlation analysis showed that CysC,nt-proBNP,TBIL and cardiac function grading were positively correlated with poor prognosis of patients with CHF.Conclusion:The grade of cardiac function in patients with CHF can be judged through detecting changes in CysC,nt-proBNP and TBIL.They can also be applied as effective indexes to assess the prognosis,and provide effective bases for development,treatment and prognosis evaluation of the disease.
