RPH与PPH治疗中/重度混合痔的对比分析

Comparative Analysis of RPH and PPH in the Treatment of Moderate and Severe Mixed Hemorrhoids

目的探讨痔疮自动套扎术(RPH)和吻合器痔上黏膜环切除术(PPH)在中/重度混合痔临床治疗中的应用效果。方法将108例中/重度混合痔患者分为观察组(RPH)和对照组(PPH)各54例,观察两组患者的手术治疗效果。结果观察组患者术中出血量(15.81±2.08)mL、创口愈合时间(9.96±1.05)、疼痛VAS评分(2.33±0.65)分、Wexner评分(1.04±0.28)分、术后并发症发生率(5.56%)均低于对照组的(40.12±4.96)mL、(11.42±1.67)d、(4.74±0.83)分、(3.94±0.43)分、20.37%(P<0.05),最长收缩时间(32.84±1.16)s、最大收缩压(12.39±2.04)kPa均高于对照组的(26.05±1.24)s、(9.08±1.67)kPa(P<0.05)。结论在中/重度混合痔的临床治疗中,RPH与PPH均有良好的应用疗效,在减轻手术损伤、加快功能恢复方面,RPH的优势作用较为显著。

Objective To explore the application effect of automatic ligation of hemorrhoids(RPH)and stapler hemorrhoidectomy(PPH)in the clinical treatment of moderate and severe mixed hemorrhoids.Methods One hurdred and eight patients with moderate or severe mixed hemorrhoids were divided into observation group(RPH)and control group(PPH),with 54 cases in each group.Results In the observation group,intraoperative blood loss(15.81±2.08)mL,wound healing time(9.96±1.05),pain VAS score(2.33±0.65)points,Wexner score(1.04±0.28)points,postoperative complication rate(5.56%)Were lower than those of the control group(40.12±4.96)mL,(11.42±1.67)d,(4.74±0.83)points,(3.94±0.43)points,20.37%(P<0.05),and the longest contraction time was(32.84±1.16)s and maximum systolic blood pressure(12.39±2.04)kPa were higher than those of the control group(26.05±1.24)s and(9.08±1.67)kPa(P<0.05).Conclusion In the clinical treatment of moderate and severe mixed hemorrhoids,both RPH and PPH have a good application effect,while RPH has a significant advantage in reducing surgical injury and accelerating functional recovery.
