出院准备计划在糖尿病视网膜病变患者延续护理管理中的应用

The application of discharge preparation plan in continuous nursing management of diabetic retinopathy patients

目的:探讨出院准备计划在糖尿病视网膜病变患者延续护理管理中的应用效果。方法:选择100例行玻璃体切割加硅油填充术的糖尿病视网膜病变患者为对象进行研究,将患者随机分为实验组和对照组,每组50例。对照组给予常规护理,实验组则另加出院准备计划干预及延续性护理。出院6个月后对两组患者的治疗依从性、自我管理能力及生活评分进行对比分析。结果:实行出院准备计划及延续性护理干预后,实验组术后视力优于对照组(P<0.05);和对照组相比,实验组治疗完全依从率更高;实验组自我管理能力及特殊体位分、饮食控制分、血糖监测分、情绪管理分、药物治疗分、行为管理分均较对照组明显增高(P<0.05);实验组干预后生活质量评分高于干预前及对照组干预后(P<0.05)。结论:出院准备计划及延续性护理干预对糖尿病视网膜病变患者有很好的效果,可提高其生活质量、自我管理能力和治疗依从性,改善预后,减少再次入院率。

Objective:To explore the application effect of discharge preparation plan in continuous nursing management of diabetic retinopathy patients.Methods:100 cases(100 eyes)with diabetic retinopathy undergoing vitreous silicone oil tamponade were randomly divided into experimental group and control group with 50 cases in each group.The control group received routine nursing,while the experimental group received discharge preparation plan intervention and continuous nursing on this basis.Six months after discharge,the treatment compliance,self-management ability and quality of life of the two groups were compared.Results:After the discharge preparation plan and continuous nursing intervention,the postoperative vision of the experimental group was better than that of the control group(P<0.05).The complete compliance rate of treatment in the experimental group was higher than that in the control group(P<0.05).The self-management ability,special posture,diet control,blood sugar monitoring,emotion management,drug therapy and behavior management of the experimental group were significantly higher than those of the control group(P<0.05).The quality of life score in the experimental group after intervention was higher than that before intervention and after intervention in the control group(P<0.05).Conclusion:Implementation of discharge preparation plan and continuous nursing intervention can improve treatment compliance,self-management ability and quality of life of diabetic retinopathy patients after vitrectomy silicone oil tamponade,and reduce readmission rate.
